关炜应用柴覆疏肝饮联合针刺治疗喘证经验

摘　　要：喘证为中医内科常见的一种病证。喘证是以症状命名的疾病,既是独立性疾病,也是多种急、慢性疾病过程中的症状,多由外邪侵袭、饮食不当、情志失调、素体虚弱等原因引起,致使七情内伤,情志常首犯于肝,肝失疏泄,气机升降受阻,故发为喘证。现代医学中肺部感染、支气管哮喘、慢性喘息性支气管炎、心源性哮喘等疾病症状均可参考喘证辨治。山西省中医药研究院关炜医师认为,喘证发病与肝密切相关,肝失疏泄,气机逆乱而导致喘证。治疗此类喘症当从“肝”论治,以疏肝为主,佐以清肝、养肝、柔肝、通肝。关炜医师临证应用柴覆疏肝饮联合针刺,以求畅达肝气,肺气肃降,升降协调,气血流行,脏腑安和,喘可治也,并强调患者要调畅情志,移情易性,治疗喘证取得良好效果。文章结合案例介绍关炜医师从肝论治喘证经验,以期为临床提供参考。Asthmatic syndrome is a common disease in TCM internal medicine.Asthmatic syndrome is a disease named after symptoms.It is not only an independent disease,but also a symptom in the process of various acute and chronic diseases.It is mainly caused by external pathogens,improper diet,emotional disorder,frail and other reasons.Resulting in internal injuries of the seven emotions,emotional will often be the first offense in the liver,liver loss and leakage,and the rise and fall of the Qi(气)machine is blocked,so it is a wheezing certificate.In modern medicine,the symptoms of pulmonary infection,bronchial asthma,chronic asthmatic bronchitis,cardiogenic asthma and other diseases can be distinguished and treated by referring to asthmatic differentiation.Mr.GUAN Wei,a doctor from Shanxi Institute of Traditional Chinese Medicine,believes that the onset of asthmatic syndrome is closely related to liver.The drainage function of liver is abnormal,and Qi movement is in disorder,leading to asthmatic syndrome.This kind of asthmatic syndrome should be treated from the perspective of liver,mainly by soothing Qi in the liver,accompanied by liver cleansing,liver nourishment,soft liver,liver circulation.Doctor GUAN Wei used the Chaifu Shugan decoction(柴覆疏肝饮)plus acupuncture to regulate Qi in the liver,in order to unblock liver Qi,lung Qi purification,lifting coordination,Qi and blood epidemic,internal organs are peaceful,asthmatic syndroma can be cured,and emphasize the patient's smooth emotional will,empathy and transposition.and achieved good results in the treatment of asthmatic syndrome.In this article,Mr.GUAN Wei’s experience in treating asthmatic syndrome from the perspective of liver is introduced with the empirical case analysis,with a view to providing a reference for clinical practice.
