徐慧军应用柴桂干姜汤治疗胆热脾寒型多囊卵巢综合征致排卵障碍性不孕症的经验

摘　　要：多囊卵巢综合征是一类常见的生殖内分泌代谢性疾病,患者多表现为月经异常、多毛、痤疮、不孕等症状,且此病发病率呈现出逐年递增的趋势。多囊卵巢综合征以高雄激素、排卵功能障碍和多囊卵巢为主要特征,发病呈现出多因性和临床表现多样性。中医根据其临床表现可将其归为“月经后期”“月经过少”“闭经”“不孕症”等的范畴,据其排卵障碍可将其归为“不孕症”。山东省青岛市中医医院妇一科徐慧军主任对多囊卵巢综合征致不孕症有多年的治疗经验,对多囊卵巢综合征理解深刻,有着独到的见解。导师认为少阳郁热,中焦寒湿,水气搏结乃本病之关键,气机升降失常,有形实邪深深伏于胞宫胞络,加之肝脾功能失调引起的一系列致病因素,导致患者出现卵泡生长、发育及排卵障碍,输卵管蠕动失司,子宫内膜容受性发生改变,从而出现不孕。徐慧军导师常使用调周法,根据月经周期内分泌所在时期的不同,将其分为行经期、经后期、经间期、经前期等四个时期。文章结合案例,从病因病机、治疗思想及用药经验等方面介绍徐慧军治疗多囊卵巢综合征致不孕症的临床经验,以期为临床提供参考。Polycystic ovary syndrome(PCOS)is a kind of common reproductive endocrine metabolic disease.The patients mostly show abnormal menstruation,hirsutism,acne,infertility and other symptoms.The incidence rate shows an increasing trend year by year.PCOS is characterized by hyperandrogenism,ovulation dysfunction and polycystic ovary,and its pathogenesis is multifactorial and its clinical manifestations are diverse.According to its clinical manifestations,TCM can classify it as late menstrual period,oligomenorrhea,amenorrhea,infertility,etc.,and according to its ovulation disorders,it can be classified as infertility.XU Huijun,a director of the First Department of Gynaecology of Qingdao Hospital of Traditional Chinese Medicine in Shandong Province,has many years of experience in the treatment of infertility caused by PCOS,and has a profound understanding of PCOS.The instructor believes that Shaoyang Yure(少阳郁热),medium-jiao(中焦)cold and humid,water and Qi(气)beating is the key to this disease,Qi machine lifting and lowering disorders,tangible real evil deep in the uterine cell network,coupled with a series of pathogenic factors caused by liver and spleen dysfunction,resulting in follicle growth,development and ovulation disorders,tubal peristalsis,endometrial receptivity changes,resulting in infertility.Tutor XU Huijun often uses the weekly adjustment method,which is divided into four periods:menstrual period,postmenstrual period,intermenstrual period,and premenstrual period according to the different periods of the endocrine period of the menstrual cycle.This article introduces XU Huijun’s clinical experience in treating infertility caused by PCOS from the aspects of etiology and pathogenesis,treatment ideas and medication experience,in order to provide a reference for clinical practice.
