摘  要:目的:研究益肾调督针法对脑缺血再灌注损伤大鼠海马神经元凋亡的影响。方法:SD雄性大鼠随机分为假手术组、模型组和益肾调督针法组,每组10只。除假手术组外,其余各组均应用改良线栓法制备大鼠大脑中动脉闭塞再灌注模型。益肾调督针法组予针刺百会、风府、大椎、命门及双侧肾俞和太溪穴,于缺血再灌注24 h后针刺1次。应用Zea-Longa评分法评估各组大鼠神经功能,应用HE染色观察大鼠海马神经细胞病理形态学变化,应用TTC染色法观察大鼠脑组织梗死情况,TUNEL法检测海马神经细胞的凋亡情况,免疫荧光技术检测JNK、p-JNK的表达情况。结果:与假手术组比较,模型组的神经行为学评分显著升高,差异具有统计学意义(P<0.01),大鼠海马神经细胞排列散乱,核固缩、深染,脑组织相对梗死体积百分比增大,差异具有统计学意义(P<0.05),凋亡神经元数量、JNK及p-JNK阳性表达均明显升高,差异具有统计学意义(P<0.05);与模型组比较,益肾调督针法组大鼠的神经行为学评分显著降低,差异具有统计学意义(P<0.01),海马神经细胞形态改善,脑组织相对梗死体积百分比明显缩小,差异具有统计学意义(P<0.05),凋亡神经细胞数目、JNK及p-JNK阳性表达明显降低,差异具有统计学意义(P<0.05)。结论:益肾调督针法能有效改善缺血再灌注大鼠神经功能缺损情况,下调海马区JNK和p-JNK的表达,减少神经细胞凋亡,这可能是其防治脑缺血再灌注损伤的机制。Objective:To study the effect of Yishen Tiaodu(tonifying Kidney and regulating Du)acupuncture on hippocampal neuron apoptosis in rats with cerebral ischemia-reperfusion injury(CIRI).Methods:SD male rats were randomly divided into the sham operation group,the model group and the acupuncture group,with 10 rats in each group.The CIRI model of middle cerebral artery occlusion was established by modified suture occlusion method.The acupuncture group was treated with needling Baihui(DU20),Fengfu(DU16),Dazhui(DU14),Mingmen(DU4)and bilateral Shenshu(BL23)and Taixi(KI3).The treatment was once 24 h after ischemia reperfusion.Zea-longa scoring method was used to evaluate neurological function,HE staining was used to observe the pathological changes of hippocampal nerve cells,TTC staining was applied to observe cerebral infarction,TUNEL method was applied to detect apoptosis of hippocampal nerve cells,and immunofluorescence technique was applied to detect expressions of JNK and p-JNK.Results:Compared with those in the sham operation group,the neurobehavioral score was significantly increased(P<0.01);the arrangement of hippocampal nerve cells was disordered,the nucleus was constricted and hyperstained,and the percentage of relative infarct volume of brain tissues was increased(P<0.05);the number of apoptotic neurons and the positive expressions of JNK and p-JNK were significantly increased in the model group(P<0.05).Compared with those in the model group,the neurobehavioral score was significantly decreased(P<0.01);the morphology of hippocampal nerve cells was improved and the relative infarct volume percentage of brain tissues was significantly decreased(P<0.05);the number of apoptotic nerve cells and the positive expressions of JNK and p-JNK were significantly decreased in the acupuncture group(P<0.05).Conclusion:Yishen Tiaodu acupuncture can effectively improve the neurological defects of ischemic and reperfusion rats,reduce the expressions of JNK and p-JNK in hippocampus,and reduce the apoptosis of nerve cells,which may b
