摘  要:目的观察分层应变技术用于评估青蒿素保护砷中毒大鼠心功能的价值。方法将32只大鼠随机分为砷中毒组、青蒿素低剂量干预组(低干预组)及高剂量干预组(高干预组)及对照组各8只。以NaAsO2对砷中毒组、低及高干预组大鼠灌胃建模;12周后对低、高干预组分别给予青蒿素30、60 mg/kg体质量,其余2组给予等量生理盐水,均连续灌胃2周。比较末次干预后各组心脏常规超声参数、分层应变参数、心肌损伤标识、炎症因子水平及心脏病理改变的差异;以Spearman相关性分析评估分层应变参数与实验室指标的相关性。结果相比对照组,另外3组左心室短轴缩短率(LVFS)、左心室射血分数(LVEF)、左心室乳头肌水平,心内膜下、中层、心外膜下心肌整体圆周应变(GCS-Endo、GCS-Mid、GCS-Epi),以及心肌各节段圆周应变、超氧化物歧化酶(SOD)、谷胱甘肽(GSH)均降低(P均<0.05);肌肉型肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)均升高(P均<0.05)。相比砷中毒组,低、高干预组LVEF、GCS-Endo,后壁、下壁、前间隔圆周应变及SOD、GSH均升高,CK-MB、LDH、TNF-α及IL-6均降低(P均<0.05);高干预组LVFS、前壁圆周应变均升高(P均<0.05)。相比低干预组,高干预组LVEF、GCS-Endo,前壁、后壁、下壁圆周应变,及SOD、GSH均升高(P均<0.05);CK-MB、LDH、TNF-α及IL-6均降低(P均<0.05)。GCS-Endo、GCS-Mid、GCS-Epi与CK-MB及LDH均呈负相关(r为-0.651~-0.865,P均<0.05),与SOD及GSH均呈正相关(r为0.642~0.880,P均<0.05)。砷中毒组大鼠心肌细胞变形,排列紊乱,细胞核不同程度萎缩、溶解;低、高干预组大鼠心肌细胞改变则有不同程度缓解。结论分层应变技术可用于评估青蒿素对砷中毒大鼠心肌的保护作用。Objective To observe the value of stratified strain technique for evaluating protective effect of artemisinin on myocardium in rats with arsenic poisoning.Methods Thirty-two rats were randomly divided into arsenic poisoning group,artemisinin low-dose intervention group(low intervention group),high-dose intervention group(high intervention group)and control group(each n=8).Rats in arsenic poisoning group,low and high intervention group were given NaAsO 2 intragastric administration for modeling.After 12 weeks,30 mg/kg body mass and 60 mg/kg body mass artemisinin was given to rats in low and high intervention group,respectively,while the same amount of saline,all through continuous gavage for 2 weeks.Then conventional ultrasound parameters,stratified strain parameters,markers of myocardial injury,levels of inflammatory factors and cardiac pathological changes were compared among groups after the last intervention.Spearman correlation analysis was used to explore the correlations of stratified strain parameters and laboratory indexes.Results Compared with control group,left ventricular fractional shortening(LVFS),left ventricular ejection fraction(LVEF),global circumferential strain in subendocardial,middle,subepicardial myocardium(GCS-Endo,GCS-Mid,GCS-Epi),and circumferential strain of all segments of myocardium at the level of left ventricular papillary muscle,as well as superoxide dismutase(SOD)and glutathione(GSH)decreased(all P<0.05),while creatine kinase isoenzymes-MB(CK-MB),lactate dehydrogenase(LDH),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)increased(all P<0.05)in arsenic poisoning group,low and high intervention groups.Compared with arsenic poisoning group,LVEF,GCS-Endo and circumferential strain of posterior wall,lower wall and anterior septal,as well as SOD and GSH increased(all P<0.05),while CK-MB,LDH,TNF-α and IL-6 decreased(all P<0.05)in low and high intervention groups.Compared with arsenic poisoning group,LVFS and circumferential strain of the anterior wall increased in high interventio
