改良Henry入路切开复位钢板内固定术治疗桡骨远端骨折的疗效研究

摘　　要：目的探讨切开复位钢板内固定术中使用改良Henry入路治疗桡骨远端骨折的临床疗效。方法选择2020年1月至2021年12月在湖州市第一人民医院行切开复位钢板内固定术的桡骨远端骨折48例患者为研究对象,依据随机数表法,分为对照组与研究组,每组24例。对照组选择直接掌侧入路,研究组选择改良Henry入路。术后6个月,比较两组腕关节活动度、上肢功能损伤(DASH)评分及并发症发生率。结果术前,两组腕关节活动度、DASH评分相比,差异无统计学意义(P>0.05);术后6个月,研究组背伸、掌屈、旋后、旋前活动度明显高于对照组,DASH评分明显低于对照组,差异有统计学意义(P<0.05);研究组并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论切开复位钢板内固定术中使用改良Henry入路治疗桡骨远端骨折可明显改善患者腕关节活动度及上肢功能,且并发症发生率较低,值得推广应用。Objective To investigate the curative effect of modified Henry's approach open reduction and internal fixation in the treatment of distal radius fractures.Methods A total of 48 patients with distal radius fractures who underwent open reduction and plate internal fixation in our hospital from January 2020 to December 2021 were selected as the research subjects.According to the random number table method,they were divided into control group and research group,with 24 patients in each group.The direct volar approach was chosed in the control group,and the modified Henry approach in the study group.6 months after operation,the range of motion of wrist joint,DASH score and complication rate were compared between the two groups.Results Before surgery,there was no significant difference in the range of motion of wrist joint between the two groups(P>0.05).6 months after surgery,the range of motion of dorsiflexion,palm flexion,supination and pronation in the study group were significantly higher than those in the control group(P<0.05).Before operation,there was no significant difference in DASH score between the two groups(P>0.05).6 months after operation,the DASH score of the study group was significantly lower than that of the control group(P<0.05).The incidence of complications in the study group was significantly lower than that in the control group(P<0.05).Conclusion Modified Henry's approach used in the open reduction and internal fixation for distal radius fractures can significantly improve the range of motion of the wrist joint and upper limb function,and the incidence of complications is low.
