和肽素、D-二聚体及血钠水平与急性肺栓塞患者病情严重程度及预后的关系

摘　　要：【目的】探讨和肽素(Copeptin)、D-二聚体(D-D)及血钠水平与急性肺栓塞患者病情严重程度及预后的关系。【方法】选取本院收治的84例急性肺栓塞患者为观察组,另外选取同期健康体检者84例为对照组。比较两组Copeptin、D-D及血钠水平;比较观察组肺栓塞严重程度指数(PESI)不同分级患者的Copeptin、D-D及血钠水平,分析观察组预后情况并比较不同预后患者Copeptin、D-D及血钠水平,分析急性肺栓塞患者Copeptin、D-D及血清钠水平与病情严重程度及预后的相关性。【结果】观察组Copeptin、D-D水平均高于对照组(P<0.05),血钠水平低于对照组(P<0.05)。PESI分级显示,观察组84例患者中,Ⅰ级22例,Ⅱ级30例,Ⅲ级20例,Ⅳ级12例,无Ⅴ级;随着PESI分级增加,患者Copeptin、D-D水平也逐渐增加,血钠水平逐渐降低,两两比较差异均有统计学意义(P<0.05)。观察组84例患者中,有9例(10.71%)患者死亡(死亡组),有75例(89.29%)患者存活(存活组);死亡组Copeptin、D-D水平均高于存活组(P<0.05),血钠水平低于存活组(P<0.05)。经Spearman检验,急性肺栓塞患者Copeptin、D-D与病情严重程度、预后呈正相关(P<0.05),血钠水平与病情严重程度、预后呈负相关(P<0.05)。【结论】急性肺栓塞患者Copeptin、D-D水平明显升高,血钠水平明显降低,且与患者病情严重程度及预后相关。【Objective】To investigate the relationship of the severity and prognosis of acute pulmonary embolism(APE)with levels of Copeptin,D-dimer,and blood sodium.【Methods】A total of 84 patients diagnosed with acute pulmonary embolism in our hospital were selected as the observation group,and 84 healthy individuals undergoing physical examination during the same period were selected as the control group.The levels of Copeptin,D-dimer(D-D),and blood sodium were compared between the two groups.According to the Pulmonary Embolism Severity Index(PESI),the levels of Copeptin,D-D,and blood sodium in patients with different severity grades were compared in the observation group.The prognosis of the observation group was analyzed,and the levels of Copeptin,D-D,and blood sodium were compared among patients with different prognosis.The relationship of Copeptin,D-D and blood sodium levels with the severity and prognosis of acute pulmonary embolism was analyzed.【Results】The levels of Copeptin and D-D in the observation group were significantly higher than those in the control group(P<0.05),while the blood sodium level was lower than that in the control group(P<0.05).According to the PESI grading,there were 22 cases of gradeⅠ,30 cases of gradeⅡ,20 cases of gradeⅢ,12 cases of gradeⅣ,and no cases of gradeⅤin the 84 patients in the observation group.With the increase in PESI grade,the levels of Copeptin and D-D gradually increased,while the blood sodium level gradually decreased.The differences between groups were statistically significant(P<0.05).Among the 84 patients in the observation group,9 patients(10.71%)died in the death group and 75 patients(89.29%)survived in the survival group.The levels of Copeptin and D-D in the death group were higher than those in the survival group(P<0.05),while the blood sodium level was lower than that in the survival group(P<0.05).Spearman's correlation analysis showed that Copeptin and D-D levels in patients with acute pulmonary embolism were positively correlated with the severi
