摘  要:目的 探讨原发性高血压患者眼脉络膜结构参数的变化。方法 回顾性收集2021年10月至2022年10月在首都医科大学附属北京友谊医院就诊的初治原发性高血压的170例患者(178眼)作为高血压组,并收集178名健康受试者(178眼)作为对照组。所有研究对象均行常规眼科检查,获得EDI-OCT图像,分别于黄斑中央凹下,中心凹鼻侧1 500μm,中央凹颞1 500μm处进行脉络膜厚度(CT)测量,利用Image-J软件对EDI-OCT图像进行二值化处理,计算脉络膜总面积(TCA)、脉络膜基质面积(SA)、脉络膜血管面积(LA)及脉络膜血管指数(CVI),比较两组CT、TCA、SA、LA和CVI的差异,分析脉络膜结构参数和舒张压及收缩压的相关性。结果 高血压组的SFCT、中心凹鼻侧CT、中心凹颞侧CT分别为(255.37±57.81)、(241.28±39.12)、(226.65±39.87)μm,均低于对照组[(271.64±67.35)、(253.71±51.17)、(236.53±41.28)μm],差异均有统计学意义(P<0.05)。高血压组的TCA、SA均低于正常组,但差异均无统计学意义(P>0.05),高血压组的LA及CVI分别为(0.527±0.170)mm^(2)、(67.47±2.95)%,均明显低于正常组[(0.563±0.140)mm^(2)、(68.69±3.37)%],差异均有统计学意义(P<0.05)。高血压组患者眼部SFCT、中心凹鼻侧CT、中心凹颞侧CT、LA及CVI与收缩压呈负相关性(r=-0.314、-0.282、-0.338、-0.437、-0.481,P<0.05),与舒张压也呈负相关性(r=-0.307、-0.275、-0.313、-0.417、-0.406,P<0.05);TCA及SA与收缩压、、舒张压均无相关性(P>0.05)。结论 原发性高血压患者眼的脉络膜结构发生变化,CT、LA及CVI降低,提示原发性高血压患者的脉络膜组织发生了缺血改变。Objective To explore the changes of Choroid structural parameters in patients with essential hypertension.Methods A total 170 patients(178 eyes)with primary hypertension who were initially treated in Beijing Friendship Hospital,Capital Medical University from October 2021 to October 2022 were retrospectively collected as the hypertension group,and 178 healthy subjects(178 eyes)were collected as the control group.All patients underwent routine ophthalmic examination,and Enhanced depth imaging optical coherence tomography(EDI-OCT)images were obtained.The choroidal thickness(CT)was measured at the fovea,1500μm nasal to the fovea and 1500μm temporal to the fovea,respectively.Image-J software was used for binarization processing of EDI-OCT images.The total choroidal area(TCA),stromal area(SA),luminal area(LA)and choroidal vascular index(CVI)were measured.The correlation between choroidal structural parameters and the diastolic and systolic blood pressure were analyzed.Results The SFCT,foveal nasal CT,and foveal temporal CT of the hypertension group were(255.37±57.81),(241.28±39.12),and(226.65±39.87)μm,respectively,which were lower than those of the control group[(271.64±67.35),(253.71±51.17),(236.53±41.28)]μm],the differences were statistically significant(P<0.05).The LA,CVI of hypertension group were(0.527±0.170)mm^(2),(67.47±2.95)%,which were lower than those of control group[(0.563±0.140)mm^(2),(68.69±3.37)%],and the differences were statistically significant(P<0.05).The ocular SFCT,foveal nasal CT,foveal temporal CT,LA,and CVI of patients with hypertension were negatively correlated with systolic blood pressure(r=-0.314,-0.282,-0.338,-0.437,-0.481,P<0.05),and also negatively correlated with diastolic blood pressure(r=-0.307,-0.275,-0.313,-0.417,-0.406,P<0.05);TCA and SA were not correlated with systolic and diastolic blood pressure(P>0.05).Conclusion The choroid structure of eyes in patients with essential hypertension is changed,and the choroidal thickness,LA and CVI are reduced,suggesting that the
