摘　　要：目的 观察鱼腥草滴鼻液联合鼻用糖皮质激素对真菌球型鼻窦炎(FBS)功能性鼻内镜手术(FESS)术后鼻腔黏膜恢复的作用。方法 前瞻性选取2019年5月至2022年10月仙桃市第一人民医院收治的180例FBS患者为研究对象,采用奇偶数分组法分为对照组88例和观察组92例。两组患者均给予FESS治疗,对照组术后给予鼻用糖皮质激素治疗,观察组术后在鼻用糖皮质激素基础上联合鱼腥草滴鼻液治疗。比较两组鼻黏膜整体重塑情况、鼻黏膜功能及鼻黏膜组织视野炎症细胞、EOS个数、不良反应情况的差异。评估两组视觉模拟评分法(VAS)评分、鼻腔鼻窦结局测试20(SNOT-20)评分等相关评分。结果 观察组鼻黏膜整体重塑情况优于对照组,差异有统计学意义(P>0.05)。治疗后,两组的VAS评分、SNOT-20评分、鼻内镜下LundKennedy评分(L-K)评分、鼻窦CT Lund-Mackay(L-M)评分、鼻黏膜组织视野炎症细胞、鼻黏膜组织视野EOS个数、鼻气道阻力均较治疗前下降,且观察组分别为(2.21±0.32)分、(9.15±1.96)分、(4.63±1.02)分、(6.52±0.57)分、(23.36±4.11)个、(12.77±3.02)个、(1.21±0.24) kPa·s^(-1)·L^(-1),均分别低于对照组[(3.74±0.36)分、(13.54±2.72)分、(6.29±1.28)分、(8.47±0.69)分、(27.54±5.03)个、(17.56±3.36)个、(1.72±0.36) kPa·s^(-1)·L^(-1)],差异均有统计学意义(P<0.05)。治疗后,观察组和对照组的鼻黏液纤毛清除率、鼻黏液纤毛清除速度均较治疗前升高,且观察组分别为(70.14±7.58)×10^(-2)、(6.43±0.89) mm/min,均分别高于对照组[(59.98±6.33)×10^(-2)、(5.58±0.87)mm/min],差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 鱼腥草滴鼻液联合鼻用糖皮质激素可促进FBS患者FESS术后鼻黏膜功能的恢复,抑制鼻黏膜组织炎症反应,且不增加不良反应。Objective To observe the effect of Houttuynia Cordata nasal drops combined with glucocorticoids on the recovery of nasal mucosa after functional endoscopic sinus surgery(FESS)for fungal bulb sinusitis(FBS).Methods A total of 180 cases of FBS patients admitted to the Xiantao First People's Hospital from May 2019 to October 2022 were prospectively selected as the research objects,and they were divided into the control group of 88 cases and the observation group of 92 cases by odd even number grouping method.Both groups were treated with FESS,the control group was treated with nasal glucocorticoid after surgery,and the observation group was treated with nasal glucocorticoid combined with Houttuynia Cordata nasal drops after surgery.The overall remodeling of nasal mucosa,the function of nasal mucosa,the number of visual field inflammatory cells and EOS,the adverse reactions were compared between the two groups.Visual analog scale(VAS)score,nasal sinus Outcome Test 20(SNOT-20)score and other relevant scores were evaluated in both groups.Results The overall remodeling of the nasal mucosa in the observation group was better than that in the control group,the difference was statistically significant(P>0.05).After treatment,the VAS score,SNOT-20 score,LundKennedy score under nasal endoscopy(L-K)score,sinus CT Lund Mackay(L-M)score,inflammatory cells in the nasal mucosa tissue field,the number of EOS in the nasal mucosa tissue field,and nasal airway resistance of both groups were lower than those before treatment,and those in the observation group were(2.21±0.32)points,(9.15±1.96)points,(4.63±1.02)points,(6.52±0.57)points,(23.36±4.11)pieces,and(12.77±3.02)pieces,(1.21±0.24)kPa·s^(-1)·L^(-1),respectively,which were lower than those in the control group[(3.74±0.36)points,(13.54±2.72)points,(6.29±1.28)points,(8.47±0.69)points,(27.54±5.03)pieces,(17.56±3.36)pieces,and(1.72±0.36)kPa·s^(-1)·L^(-1)],the differences were statistically significant(P<0.05).After treatment,the nasal mucociliary clearance rate and na
