SPECT/CT唾液腺动态显像对干燥综合征诊治的临床研究

摘　　要：目的:探讨SPECT/CT唾液腺动态显像在干燥综合征诊治中的临床价值。方法:收集2021年3月1日至2023年2月28日期间于我院临床诊断符合2002年SS国际分类(诊断)标准的干燥综合征患者,总共116例(观察组),选择无唾液腺疾病的健康查体者20例(对照组)。探讨SPECT/CT唾液腺动态显像在干燥综合征中的诊断效能,计算敏感度、特异度、准确度。评估唾液腺受损程度,记录分泌率(MSR)、相对摄取率(S/I)、摄取指数(UR)及酸刺激后唾液腺放射性计数由高峰降至最低时间(Tmin)。结果:以临床诊断结果为金标准,SPECT/CT唾液腺动态显像诊断SS敏感度95.69%(111/116)、特异度90.00%(18/20)、准确率94.85%(129/136)。观察组患者中,唾液腺功能轻度受损者50例,唾液腺功能中度受损者38例,唾液腺功能重度受损者23例,唾液腺功能正常5例;组间MSR、S/I、UR水平对比,观察组明显低于对照组(P<0.05);组间Tmin水平对比,观察组明显高于对照组(P<0.05);中度受损组、重度受损组患者的MSR、S/I、UR水平低于轻度受损组(P<0.05);中度受损组、重度受损组患者的Tmin水平高于轻度受损组(P<0.05);重度受损组患者的MSR、S/I、UR水平低于中度受损组(P<0.05);重度受损组患者的Tmin水平高于中度受损组(P<0.05)。结论:SPECT/CT唾液腺动态显像在干燥综合征诊断中具有较佳的诊断效能,可以有效评估唾液腺受损程度,为患者的治疗提供影像学依据。Objective:To explore the clinical value of SPECT/CT salivary gland dynamic imaging in the diagnosis and treatment of Sjogren's syndrome.Methods:A total of 116 patients with Sjogren's syn⁃drome(SS)who were clinically diagnosed in our hospital from March 1,2021 to February 28,2023 and met the 2002 SS international classification(diagnostic)criteria were collected as observation group and 20 healthy individuals without salivary gland disease were selected as control group.To investigate the di⁃agnostic efficacy of SPECT/CT salivary gland dynamic imaging in Sjogren's syndrome,and calculate sen⁃sitivity,specificity,and accuracy.Assess the degree of damage to the salivary glands,record maximum secretion ratio(MSR),relative uptake rate(S/I),uptake ratio(UR),and the time(Tmin)when the salivary gland radiation count decreases from peak to minimum after acid stimulation.Results:Using clinical diag⁃nostic results as the gold standard,the sensitivity of SPECT/CT salivary gland dynamic imaging in diag⁃nosing SS was 95.69%(111/116),the specificity was 90.00%(18/20),and the accuracy was 94.85%(129/136).In the observation group,there were 50 patients with mild damage to salivary gland function,38 pa⁃tients with moderate damage of salivary gland function,23 patients with severe damage of salivary gland function,and 5 patients with normal salivary gland function;The comparison of MSR,S/I,and UR levels between groups showed that the values of observation group were significantly lower than those of control group(P<0.05);The comparison of Tmin levels between groups showed that the values of observation group was significantly higher than those of control group(P<0.05);The levels of MSR,S/I,and UR in pa⁃tients with moderate and severe damage were lower than those in patients with mild damage(P<0.05);The Tmin levels in patients with moderate and severe damage were higher than those in patients with mild damage(P<0.05);The levels of MSR,S/I,and UR in the severely damaged group were lower than those in the moderately damag
