摘 要:目的:探索集成MRI定量评价腰椎间盘退变的应用价值。方法:92名腰痛患者和20名志愿者接受常规MRI与集成MRI扫描,用Pfirrmann标准对椎间盘分级,测量椎间盘的T1、T2、PD值。用方差分析及事后检验对各级别椎间盘不同部位的T1、T2、PD值比较。有序回归分析T1、T2、PD值在综合评估椎间盘退变时的作用。构建ROC曲线,计算AUC、临界值、P值,比较有效值间AUC的差异。结果:腰痛组460个椎间盘,对照组115个椎间盘。除前后纤维环的PD值外,Pfirrmann分级与余部位的T1、T2、PD值呈负相关。前后纤维环T2值、髓核T1、T2、PD值在各级间有差异。Ⅰ-II级,前纤维环T2值、髓核T1、T2、PD值有意义;Ⅱ-Ⅲ级,髓核T1、T2、PD值有意义;Ⅲ-Ⅳ级,髓核T2值有意义。对Ⅰ-II级AUC,髓核T2值高于T1、PD值;Ⅱ-Ⅲ级AUC,髓核T2值高于T1、PD值,髓核T1值高于PD值。Ⅲ-Ⅳ级AUC,髓核T2值高于PD值。结论:集成MRI对腰椎间盘退变的临床研究有应用价值。Objective:To explore the application value of integrated MRI in quantitative evaluation of lumbar intervertebral disc degeneration.Methods:92 patients with low back pain and 20 volunteers were scanned by conventional MRI and integrated MRI.The intervertebral discs were graded by Pfirrmann standards,and the values of T1,T2 and PD of intervertebral discs were measured.The T1,T2 and PD val⁃ues of different parts of intervertebral discs were compared by analysis of variance and post-test.The role of T1,T2 and PD values in comprehensive evaluation of intervertebral disc degeneration was analyzed by ordered regression analysis.Construct ROC,calculate AUC,critical value and P value,and compare the difference of AUC among effective values.Results:There were 460 intervertebral discs in low back pain group and 115 intervertebral discs in control group.Except for the PD value of the anulus fibrosus,the Pfirrmann grade was negatively correlated with the T1,T2 and PD values of the other parts.There were differences in T2 values of anterior and posterior annulus fibrosus,T1,T2 and PD values of nucleus pulposus.For I-II grade,T2 value of anterior fibrous annulus,T1,T2,PD value of nucleus pulposus were meaningful;for II-III grade,T1,T2,PD value of nucleus pulposus were meaningful;for III-IV grade,T2 value of nucleus pulposus were meaningful.For I-II AUC,the T2 value of nucleus pulposus was higher than T1 and PD,and for II-III AUC,the T2 value of nucleus pulposus was higher than T1 and PD,and the T1 value of nucleus pulposus was higher than PD value.For III-IV grade AUC,T2 value of nucleus pulposus is higher than PD value.Conclusion:Integrated MRI has application value in clinical study of lumbar intervertebral disc degeneration.
