摘  要:目的:研究腓肠肌内侧头训练伤的核磁共振成像(MRI),探讨损伤的影像学特点和MRI应用价值。方法:回顾性分析48例腓肠肌内侧头损伤的MRI影像学资料,分析损伤组织影像学改变和特点,将腓肠肌损伤进行分级评价。结果:本组腓肠肌组织损伤19例,MRI表现为腓肠肌软组织水肿,信号不均匀。其中,Ⅰ损伤占68.4%,Ⅱ级、Ⅲ级损伤分别占15.8%。肌筋膜损伤水肿和组织间隙积液28例,表现为筋膜水肿增厚,信号不规则,走形失常。间隙积液多延组织间隙分布,成梭形或带状异常信号。肌腱和腱鞘损伤26例,表现为肌腱局部肿胀增厚、腱纤维不连续以及腱内信号混杂。腱止点末端病6例,表现为腱止点区骨髓水肿呈不均匀局限高信号,周围骨质硬化,软组织增厚。结论:腓肠肌内侧头训练损伤常见,MRI对病变显示和分级诊断具有重要的应用价值。Objective:To study the magnetic resonance imaging(MRI)of medial head training injury of gastrocnemius muscle to investigate the imaging characteristics of injury and MRI application value.Methods:Retrospectively reviewed the MRI imaging data of 48 patients with medial head injury of the gastrocnemius,and analyzed the imaging changes and characteristics of the injury tissue,and graded the gastrocnemius injury.Results:In this group,there were 19 cases of gastrocnemius muscle tissue injury,and MRI showed gastrocnemius muscle soft tissue edema with uneven signal.Among them,68.4%had gradeⅠinjury,while 15.8%had gradeⅡandⅢinjury,respectively.Myofascial injury edema and tissue space effusion were observed in 28 cases,which showed edematous thickening of fascia with irregular sig⁃nal and vagus deformity.The interstitial effusions were multilayered with an interstitial distribution and abnormal signal in fusiform or zonal pattern.Tendon and tendon sheath injuries were described in 26 pa⁃tients and showed focal swelling and thickening of tendons,discontinuity of tendon fibers,and confounding of intratendinous signals.Tendinous end disease was present in 6 patients and showed heterogeneous localized hyperintensity with bone marrow edema in the tendinous end area,surrounding osteosclerosis,and soft tissue thickening.Conclusions:Medial head training injuries of the gastrocnemius are common,and MRI has important diagnostic applications for lesion display and graded diagnosis.
