摘　　要：目的研究Padua评分对老年住院病人发生静脉血栓栓塞症(VTE)的预测价值。方法回顾性分析杭州市第三人民医院全科医学科2018年1月至2021年6月收治的150例老年住院病人,完善CT肺动脉造影(CTPA)和(或)双下(上)肢静脉超声检查,根据检查结果将病人分为VTE组和非VTE组,收集2组病人的基线资料,包括年龄、性别、基础疾病、Padua评分、D-二聚体(DD)、平均血小板体积(MPV)、白蛋白并进行比较。采用多因素Logistic回归分析发生VTE的危险因素。采用AUC评估Padua评分预测老年住院病人发生VTE的效能。结果VTE组和非VTE组间Padua评分、年龄、高血压、急性感染、DD、MPV以及白蛋白水平比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,≥85岁是老年住院病人发生VTE的独立危险因素。Padua评分预测老年住院病人发生VTE的AUC为0.631,最佳截断值≥4.5分,约登指数为0.36。结论Padua评分对老年病人发生VTE的临床预测价值有限。Objective To assess the predictive value of Padua score foRvenous thromboembolism(VTE)in the elderly inpatients.Methods A retrospective analysis was performed and 150 elderly inpatients in Department of General Practice of Hangzhou Third People’s Hospital from January 2018 to June 2021 were included.All of them underwent CT pulmonary angiography oRvenous ultrasound of both upper(lower)limbs,and were divided into VTE group and non-VTE group.The baseline data including age,gender,underlying disease,Padua score,D-dimer,mean platelet volume(MPV),albumin were collected and compared between the two groups.Multivariate Logistic regression was used to analyze the risk factors of VTE.ReceiveRoperating characteristic(ROC)curve analysis was carried out to assess the predictive value of Padua score.Results There were statistically significant differences in Padua score,age,hypertension,acute infection,D-dimer,MPV and albumin between the two groups.Multivariate Logistic regression analysis showed that aged≥85 years old was the independent risk factoRfoRVTE of elderly inpatients.The area undeRthe curve(AUC)of Padua score to predict the incidence of VTE was 0.631,with a cut-off value of 4.5,and the Youden index was 0.36.Conclusions The predictive value of Padua score foRVTE in elderly inpatients is limited.
