凸面造口底盘联合湿性愈合敷料在肠造口皮肤黏膜分离中的治疗效果观察

摘　　要：目的:探讨凸面造口底盘配合湿性愈合敷料对肠造口皮肤黏膜分离的治疗效果。方法:选取2020年4月—2022年6月安徽医科大学第一附属医院门诊换药治疗的肠造口皮肤黏膜分离患者56例为观察对象,根据随机数字表法分为对照组与观察组,各28例。对照组采用常规处理方案,观察组采用凸面造口底盘配合湿性愈合敷料进行换药治疗。比较两组患者肠造口愈合指标、疼痛程度及生活质量,随访并发症发生情况。结果:观察组创面愈合时间短于对照组,换药次数、造口渗液量少于对照组,差异有统计学意义(P<0.001);治疗后1、2、4周,观察组视觉模拟评分低于对照组,差异有统计学意义(P<0.001);治疗后,两组各项生活质量评分均提高,观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P=0.029)。结论:使用凸面底盘配合湿性愈合敷料,能促进皮肤黏膜分离的创面愈合,有效缓解创面疼痛,改善患者生活质量,减少并发症的发生。Objective:To investigate the therapeutic effect of convex stoma sump with wet healing dressing on the separation of skin mucosa of enterostomies.Methods:Fifty-six patients with enterostomy skin mucosa separation treated with outpatient dressing change in the First Affiliated Hospital of Anhui Medical University from April 2020 to June 2022 were selected as observation objects,and were divided into control group and observation group,each with twenty-eight cases,according to the method of randomized numerical table.The control group was treated with conventional treatment program,and the observation group was treated with convex stoma sump with wet healing dressing for dressing change.Comparison of the two groups of intestinal patients with intestinal stoma healing indexes,pain level and quality of life,follow-up complications.Results:The wound healing time of the observation group was shorter than that of the control group,and the number of dressing changes and the amount of stoma exudate were less than that of the control group,and the difference was statistically significant(P<0.001);At 1,2 and 4 weeks after treatment,the visual analog score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.001);After treatment,all quality of life scores improved in both groups,and the observation group was higher than the control group,with a statistically significant difference(P<0.05);The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(P=0.029).Conclusion:The use of convex sump with wet healing dressing can promote wound healing of separation of skin mucosa,effectively relieve wound pain,improve the quality of life of patients,and reduce the occurrence of complications.
