低温等离子靶向消融术联合渐进性放松训练疗法治疗腰椎间盘突出症和对患者睡眠质量的影响

摘　　要：目的:探讨低温等离子靶向消融术联合渐进性放松训练疗法治疗腰椎间盘突出症(LDH)患者的疗效及对睡眠质量的影响。方法:选取2020年7月至2021年7月上海市嘉定区中医医院疼痛科收治的LDH患者90例作为研究对象,按照随机数字表法分为观察组和对照组,每组45例,观察组采用低温等离子靶向消融术配合渐进性放松训练疗法,对照组采用射频靶点热凝术配合渐进性放松训练疗法。比较2组患者临床疗效、疼痛程度和睡眠质量。结果:观察组治疗优良率为95.56%,显著高于对照组(80.00%,P<0.05);2组患者术前疼痛视觉模拟评分(VAS)、匹兹堡睡眠质量指数(PSQI)评分比较差异无统计学意义(P>0.05);术后1、3个月2组患者疼痛VAS、PSQI评分均显著降低,观察组术后1、3个月疼痛VAS、PSQI评分均显著低于对照组(P<0.05),2组LDH患者疼痛VAS、PSQI评分组间、时间及交互效应比较差异均有统计学意义(P<0.05)。结论:低温等离子靶向消融术联合渐进性放松训练疗法治疗LDH可以取得理想疗效,可显著降低患者疼痛,并提高患者睡眠质量。Objective:To investigate the effect of targeted low-temperature plasma ablation combined with progressive relaxation training in the treatment of patients with lumbar disc herniation(LDH)and the influence on sleep quality.Methods:A total of 90 patients with LDH admitted to Jiading Hospital of Traditional Chinese Medicine from July 2020 to July 2021 were selected,and divided into observation group(targeted low-temperature plasma ablation combined with progressive relaxation training)and control group(radiofrequency target thermocoagulation combined with progressive relaxation training),45 cases in each group.Clinical efficacy,pain degree and sleep quality were compared between the two groups.Results:The excellent and good rate of curative effect in observation group(95.56%)was significantly higher than that in control group(80.00%)(P<0.05).There was no statistically significant difference in Visual Analogue Scale(VAS)score or Pittsburgh sleep Quality Index(PSQI)score between the two groups before operation(P>0.05).At 1 month and 3 months after operation,the VAS and PSQI scores of the two groups were significantly decreased,and the observation group had significantly lower scores than the control group(P<0.05).There were statistically significant differences in VAS and PSQI scores in terms of inter-group effect,time effect and interaction effect(P<0.05).Conclusion:Targeted low-temperature plasma ablation combined with progressive relaxation training is effective in the treatment of LDH,which can significantly relieve pain and improve sleep quality of the patients.
