老年冠心病患者居家心脏康复参与意向现状及其影响因素分析

摘　　要：目的探讨老年冠心病患者居家心脏康复参与意向现状及其影响因素,并提出提高老年冠心病患者参与居家心脏康复意向的方案。方法采用便利抽样法纳入2022年2月1日至2023年1月25日山东省齐鲁医院就诊的冠心病患者226例,收集患者性别、年龄、文化程度、病程等一般资料及是否有参与居家心脏康复的意向,分别使用慢性病患者健康素养量表、健康行为能力自评量表、社会支持评定量表、生活质量量表(SF-12)评估患者健康素养、健康行为的能力以及社会支持情况。以老年冠心病患者是否有意向参与居家心脏康复为因变量进行二元Logistic回归分析,探究老年冠心病患者参与居家心脏康复意向的独立影响因素及其效应。结果无康复意向患者196(86.73%)例,有康复意向患者30(13.27%)例,多数老年冠心病患者无参与居家心脏康复的意向。二元Logistic回归分析结果显示,经历过经皮冠状动脉介入术治疗(OR=0.277,P<0.01)、并存其他疾病(OR=0.322,P<0.05)、住院期间接受过专业康复指导(OR=0.367,P<0.05)与老年冠心病患者参与居家心脏康复意向呈负相关,交流互动能力(OR=1.102,P<0.01)、活力状况(OR=1.054P<0.05)与老年冠心病患者参与居家心脏康复意向呈正相关。结论交流互动能力与活力的提升有助于增加老年冠心病患者的居家心脏康复参与意向,而经历过经皮冠状动脉介入术、并存其他疾病及住院期间接受过专业指导会降低老年冠心病患者的参与意向,应对该类患者进行重点干预。Objective To investigate the willingness of elderly patients with coronary heart disease to participate in home-based cardiac rehabilitation and influencing factors,and to propose a plan to improve these patients'willingness to participate in home-based cardiac rehabilitation.Methods A total of 226 patients with coronary heart disease treated at Qilu Hospital in Shandong Province between February 1,2022 and January 25,2023 were included by convenience sampling.General data of patients such as sex,age,education level,and course of disease,and whether they were willing to participate in home-based cardiac rehabilitation were collected.Patients'health literacy,health behavior ability and social support were assessed using the chronic disease patients'health literacy scale,the self-rated abilities for health practices scale,the social support assessment scale,and the 12-item short form survey.Using whether elderly patients with coronary heart disease were willing to participate in home-based cardiac rehabilitation as the dependent variable,binary logistic regression analysis was carried out to identify independent influencing factors for the willingness of elderly patients with coronary heart disease to participate in home-based cardiac rehabilitation and their impact.Results 196(86.73%)patients expressed no willingness while 30(13.27%)expressed a willingness,suggesting most elderly patients with coronary heart disease were not willing to participate in home-based cardiac rehabilitation.The results of binary Logistic regression analysis showed that experience of percutaneous coronary intervention treatment(OR=0.277,P<0.01),other concurrent diseases(OR=0.322,P<0.05),and receiving professional rehabilitation instructions during hospitalization(OR=0.367,P<0.05)were negatively correlated with the willingness of elderly patients with coronary heart disease to participate in home-based cardiac rehabilitation,whereas communication skills(OR=1.102,P<0.01)and vitality(OR=1.054,P<0.05)were positively correlated with the willin
