造口治疗师主导的多学科协作护理模式在足踝开放性损伤患者中的应用

摘　　要：目的:探讨造口治疗师为主导的多学科协作护理模式(MDT)在足踝开放性损伤患者中的应用效果。方法:选取2021年6月1日~2022年10月31日手足外科住院治疗的183例足踝开放性损伤患者为研究对象,随机分为对照组92例和观察组91例,对照组接受常规护理与健康教育,观察组在对照组基础上给予造口治疗师为主导的MDT。比较两组症状恢复情况(卧床时间、住院时间、伤口完全愈合时间、Kofoed踝关节评分)、并发症发生率(伤口感染、静脉血栓、关节僵硬、血管或神经损伤)和护理满意度。结果:观察组卧床时间、住院时间、伤口完全愈合时间均短于对照组(P<0.01),Kofoed踝关节评分高于对照组(P<0.01);观察组并发症发生率低于对照组(P<0.05);观察组护理效果满意度高于对照组(P<0.01)。结论:以造口治疗师为主导的MDT在足踝开放性损伤患者的治疗护理中起到重要作用,可有效提高患者肢体功能恢复效率,改善医患关系。Objective:To explore the effect of a multidisciplinary collaborative care model led by stoma therapists in patients with open foot and ankle injuries.Methods:From June 1,2021 to October 31,2022,183 patients treated with open hand and foot surgery were selected as the study subjects and randomly divided into the control group with 92 cases and the observation group with 91 cases.The control group received conventional care and health education,and the observation group was given a multidisciplinary collaborative care model led by an ostomy therapist on the basis of conventional care and health education.The two groups were compared in terms of symptom recovery(bed rest time,hospital stay,time to complete wound healing,Kofoed ankle score),complication rate(wound infection,venous thrombosis,joint stiffness,vascular or nerve injury)and nursing satisfaction.Results:The in bed rest time,length of hospital stay,time to complete wound healing in the observation group were shorter than those in the control group(P<0.01),and the Kofoed ankle scores in the observation group were higher than those in the control group(P<0.01).The incidence of complications in the observation group was lower than that in the control group(P<0.05).And the satisfaction with nursing in the observation group was higher than that in the control group(P<0.01).Conclusion:The multidisciplinary collaborative nursing model led by stoma therapists plays an important role in the therapeutic care of patients with open foot and ankle injuries,which can effectively improve the efficiency of their limb function recovery and promote the doctor-patient relationship.
