摘　　要：目的 研究融入冰山理论的系统性综合管理对免疫球蛋白A(IgA)肾病患者遵医行为、营养状态及肾功能的影响。方法 选取本院2020年11月至2022年11月期间112例IgA肾病患者随机分组,对照组56例给予系统性干预,观察组56例联合基于冰山理论的综合管理干预,对比两组患者遵医行为、营养状态和肾功能。结果 观察组干预后,遵医行为、面对评分均高于对照组,屈服、回避评分低于对照组(P<0.05);观察组干预后,血红蛋白(Hb)、白蛋白(ALB)水平均高于对照组(P<0.05);观察组干预后,尿素氮、尿酸、血肌酐、β2微球蛋白清除试验水平均低于对照组(P<0.05)。结论 融入冰山理论的系统性综合管理能够有效提高IgA肾病患者遵医行为,改善应对方式,促进营养状态和肾功能恢复。Objective To study the effects of systematic comprehensive management with iceberg theory on compliance behavior,nutritional status and renal function in patients with immunoglobulin A(IgA)nephropathy.Methods A total of 112 patients with IgA nephropathy from November 2020 to November 2022 were randomly grouped in our hospital.56 patients in the control group received systematic intervention,and 56 patients in the observation group combined with comprehensive management intervention based on iceberg theory.Compliance behavior,nutritional status and renal function were compared between the two groups.Results After intervention,compliance behavior and face scores in observation group were higher than those in control group,while yield and avoidance scores were lower than those in control group(P<0.05).After intervention,the levels of hemoglobin(Hb)and albumin(ALB)in observation group were higher than those in control group(P<0.05).After intervention,the levels of urea nitrogen,uric acid,creatinine andβ2 microglobulin clearance test in the observation group were lower than those in the control group(P<0.05).Conclusion Systematic comprehensive management with iceberg theory can effectively improve compliance behavior,coping style,nutritional status and renal function recovery in patients with IgA nephropathy.
