摘　　要：目的观察盆腔炎汤治疗盆腔炎性疾病所致输卵管性不孕的临床疗效及安全性。方法回顾性收集盆腔炎性疾病所致输卵管性不孕患者240例,其中采用自拟盆腔炎汤灌肠治疗80例(灌肠组),口服盆腔炎汤治疗80例(口服组),行输卵管通液术治疗80例(对照组),均治疗3个疗程。随访1年,记录妊娠时间并计算受孕率、异位妊娠发生率及不良反应。结果灌肠组、口服组受孕率分别为65.4%、31.2%,均高于对照组的20%(P均<0.05),且灌肠组高于口服组(P<0.05);灌肠组、口服组异位妊娠率分别为1.28%、2.6%,均低于对照组的4%,组间比较差异无统计学意义(P>0.05);不良反应发生率灌肠组7.69%,口服组12.99%,对照组36%。由低到高依次为灌肠组、口服组、对照组,与对照组比较差异有统计学意义(P<0.05)。结论应用自拟盆腔炎汤治疗盆腔炎性疾病所致输卵管性不孕,可提高受孕率,降低异位妊娠发生率,而且较少出现不良反应,灌肠治疗疗效优于口服治疗。Objective To observe the clinical efficacy and safety of pelvic inflammatory Decoction in the treatment of tubal infertility caused by pelvic inflammatory diseases.Methods 240 cases of tubal infertility caused by pelvic inflammatory diseases were collected retrospectively,including 80 cases treated with self-made pelvic inflammatory Decoction enema(enema group),80 cases treated with oral pelvic inflammatory Decoction(oral group)and 80 cases treated with salpingo liquid dredging(control group),all of which were treated for 3 courses.The pregnancy time was recorded and the pregnancy rate,the incidence of ectopic pregnancy and adverse reactions were calculated.Results the pregnancy rates of enema group and oral group were 65.4%and 31.2%respectively,which were higher than 20%of the control group(P<0.05);The ectopic pregnancy rates of enema group and oral group were 1.28%and 2.6%respectively,which were lower than 4%of the control group.There was no significant difference between the two groups(P>0.05);The incidence of adverse reactions was 7.69%in enema group,12.99%in oral group and 36%in control group.The order from low to high was enema group,oral group and control group,which had significant difference compared with the control group(P<0.05).Conclusion the application of self-made pelvic inflammatory Decoction in the treatment of tubal infertility caused by pelvic inflammatory diseases can improve the pregnancy rate,reduce the incidence of ectopic pregnancy,and have less adverse reactions.The curative effect of enema is better than oral treatment.
