Clinical observation of shock wave combined with isokinetic muscle training system in the treatment of 156 patients after anterior cruciate ligament injury reconstruction

摘  要:目的 分析冲击波联合等速肌力训练系统治疗膝关节前交叉韧带(ACL)损伤重建术后患者的临床疗效。方法 选取2021年1月—2022年1月在天津医科大学总医院康复医学科门诊收治的膝关节ACL损伤重建术后患者156例。采用数字随机法分为观察组和对照组,每组各78例,对照组采用脉冲磁结合常规运动训练治疗,观察组采用冲击波联合等速肌力训练系统治疗,均隔天1次,治疗2周。比较治疗前、治疗后第3、7次以及随访30 d的股直肌肌肉厚度、关节活动度ROM评分、Lysholm膝关节评分量表(LKSS)评分及膝关节等速肌力。结果 治疗后,随着治疗时间的推移,两组股直肌厚度增加(P<0.05);观察组治疗7次及30 d后的股直肌厚度大于对照组(P<0.05)。治疗后,随着治疗时间的推移,观察组ROM评分增加,对照组治疗7次后ROM评分增加;观察组治疗后3次、7次及30 d ROM评分高于对照组(P<0.05)。随着治疗时间推移,两组膝关节LKSS评分增加(P<0.05);观察组治疗7次及30 d后LKSS评分高于对照组(P<0.05)。随着治疗时间的推移,观察组屈膝关节峰力矩值峰增加(P<0.05),对照组治疗7次后屈膝关节峰力矩值增加(P<0.05),观察组治疗3次、7次及30 d后屈膝关节峰力矩值大于对照组(P<0.05);随着治疗时间的推移,观察组伸膝关节峰力矩值增加(P<0.05),对照组治疗7次后伸膝关节峰力矩值增加(P<0.05),观察组治疗3次、7次、30 d后伸膝关节力峰值矩大于对照组(P<0.05)。随着治疗时间的推移,两组肌力比值(H/Q)降低(P<0.05),观察组治疗7次及30 d后H/Q值低于对照组(P<0.05)。结论 冲击波联合等速肌力训练系统对前交叉韧带损伤重建术后患者具有较好的治疗效果。Objective To analyze the clinical effect of shock wave combined with isokinetic muscle training system in the treatment of patients with knee anterior cruciate ligament injury and reconstruction.Methods 156 patients with knee anterior cruciate ligament injury after surgery were selected.The numerical randomization method was divided into 78 cases in the experimental group and 78 cases in the control group,the control group was treated with pulsed magnetism combined with conventional exercise training,and the experimental group was treated by shock wave combined with Isokinetics muscle strength training system,once every other day,for 2 weeks.The muscle thickness(rectus femoris),joint range of motion,Lysholm Knee Score Scale and Knee Joint Isokinetics before treatment,3rd and 7th after treatment and one month(30 days)follow-up force were observed.The clinical efficacy and safety were evaluated.Results After treatment,as the treatment time passed,the thickness of the rectus femoris increased significantly(P<0.01).The knee flexion and extension activities increased significantly(P<0.01).The LKSS score Significantly improved(P<0.05).The change of knee flexion peak torque was significantly improved in the determination of isokinetic muscle strength(P<0.01),and the change of knee extension peak torque was improved(P<0.05).The hamstrings and quadriceps muscles The force ratio has been significantly improved(P<0.01).Conclusion The shock wave combined with isokinetic muscle training system has a good therapeutic effect on patients after anterior cruciate ligament injury reconstruction.
