腹腔镜下疝气针腹膜外疝囊高位结扎术治疗小儿腹股沟斜疝的效果

摘　　要：目的探究腹腔镜下疝气针腹膜外(LEHN)疝囊高位结扎术(HLHS)治疗小儿腹股沟斜疝(IIH)的效果。方法选取南阳市第二人民医院2020年4月至2022年4月收治的198例IIH患儿,采用随机数字表法分为对照组(99例)与观察组(99例)。对照组接受HLHS治疗,观察组接受LEHN-HLHS术治疗。比较两组临床疗效、手术相关指标、疼痛程度、并发症及复发情况。结果观察组临床总有效率为95.96%,高于对照组的82.83%(P<0.05)。观察组患儿手术时间、下床活动时间、排气时间、住院时间、术后疼痛时间及切口长度均短于对照组,术中出血量少于对照组(P<0.05)。术前两组患儿视觉模拟(VAS)评分比较差异无统计学意义(P>0.05),术后两组患儿VAS评分较术前均降低,且观察组低于对照组(P<0.05)。观察组并发症发生率、复发率分别为3.03%、2.02%,低于对照组的10.10%、12.12%(P<0.05)。结论与传统手术治疗相比,LEHN-HLHS术疗效更明显,对患儿各项手术指标以及疼痛改善作用较好,且术后并发症少,复发情况良好。Objective To investigate the clinical effects of laparoscopic hernia needle extraperitoneal(LEHN)high ligation of hernia sac(HLHS)in treating children with indirect inguinal hernia(IIH).Methods A total of 198 children with IIH admitted to the Second People’s Hospital of Nanyang from April 2020 to April 2022 were selected,and divided into the control group and the observation group by random number table method,with 99 cases in each group.The control group underwent HLHS,while the observation group were treated with LEHN-HLHS.Clinical effects,surgery-related indicators,pain degree,complications and recurrence were compared between the two groups.Results The total clinical response rate in the observation group(95.96%)was higher than that in the control group(82.83%)(P<0.05).Operation time,time for out-of-bed activities,exhaust time,length of hospital stay,postoperative pain duration and length of incision in the observation group were shorter than those in the control group,intraoperative blood loss was less than the control group(P<0.05).There was no statistical difference in the visual analogue scale(VAS)scores between the two groups before surgery(P>0.05).After surgery,the VAS scores of the two groups were decreased,and the VAS score of the observation group was lower than that of the control group(P<0.05).The incidence of complications and recurrence in the observation group(3.03%and 2.02%)were lower than those in the control group(10.10%and 12.12%)(P<0.05).Conclusion Compared with traditional surgical treatment,LEHN-HLHS is more effective in the treatment of children with IIH,and can improve surgical indicators and alleviate pain,with few postoperative complications and recurrence.
