吻合器痔上黏膜环切术联合外剥内扎术治疗重度脱垂性环状混合痔患者的效果

摘　　要：目的:观察吻合器痔上黏膜环切术(PPH)联合外剥内扎术(MMH)治疗重度脱垂性环状混合痔患者的效果。方法:回顾性分析2020年10月至2022年8月该院收治的80例Ⅲ~Ⅳ度脱垂性环状混合痔患者的临床资料,根据手术方案不同将其分为对照组和观察组各40例。对照组采用MMH治疗,观察组采用PPH联合MMH治疗,比较两组临床疗效、手术相关指标(术中出血量、手术时间、切口愈合时间和术后住院时间)水平、术后并发症发生率和手术前后肛门功能指标(肛管静息压、直肠静息压、肛管高压带长度、肛管最大收缩压)水平。结果:两组治疗总有效率、术后并发症发生率、术后住院时间、术后直肠静息压和肛管最大收缩压比较,差异均无统计学意义(P>0.05);观察组术中出血量少于对照组,手术时间、切口愈合时间均短于对照组,差异有统计学意义(P<0.05);术后,两组肛管静息压均高于术前,且观察组高于对照组,两组肛管高压带长度均长于术前,且观察组长于对照组,差异有统计学意义(P<0.05)。结论:PPH联合MMH治疗重度脱垂性环状混合痔患者可减少术中出血量,缩短手术时间、切口愈合时间,提高肛管静息压,延长肛管高压带长度。Objective:To effects of procedure for prolapse and hemorrhoids(PPH)combined with Milligan-Morgan hemorrhoidectomy(MMH)in treatment of patients with severe prolapsed circular mixed hemorrhoids.Methods:The clinical data of 80 patients with grade III-IV prolapsed c circular mixed hemorrhoids admitted to this hospital from October 2020 to August 2022 were retrospectively analyzed.According to different surgical schemes,they were divided into control group and observation group,40 cases in each group.The control group was treated with MMH,while the observation group was treated with PPH combined with MMH.The clinical efficacy,the operation related indicator levels(intraoperative blood loss,operation time,incision healing time and postoperative hospitalization time),the incidence of postoperative complications,and the anal function indicator levels(anal canal resting pressure,rectal resting pressure,anal high pressure zone length,anal maximal squeeze pressure)were compared between the two groups before and after the surgery.Results:There were no significant differences in the total effective rate of treatment,the incidence of postoperative complications,the postoperative hospitalization time,the postoperative rectal resting pressure and the maximal squeeze pressure between the two groups(P>0.05).The intraoperative blood loss in the observation group was less than that in the control group,the operation time and incision healing time were shorter than those in the control group,and the differences were statistically significant(P<0.05).Further,after the surgery,the anal canal resting pressure levels of the two groups were higher than those before the surgery,and that of the observation group was higher than that of the control group;the anal high pressure zone length levels in the two groups were longer than those before the surgery,and that in the observation group was longer than that in the control group;and the differences were statistically significant(P<0.05).Conclusions:PPH combined with MMH in the treatment of t
