多层CT尿路成像检查在非结石性尿路梗阻诊断中的效能

摘　　要：目的:分析多层CT尿路成像检查在非结石性尿路梗阻诊断中的效能。方法:选取2019年1月至2021年6月该院收治的92例疑似非结石性尿路梗阻患者进行前瞻性研究,所有患者均行多层CT尿路成像检查,以手术诊断结果为金标准,分析多层CT尿路成像检查在非结石性尿路梗阻诊断中的效能,并采用Kappa检验多层CT尿路成像检查与手术在非结石性尿路梗阻诊断中的一致性。结果:手术诊断结果显示,92例疑似非结石性尿路梗阻患者确诊阳性80例,包括泌尿系肿瘤28例,肿瘤侵犯或外源性压迫22例,泌尿系慢性炎症20例,先天发育畸形5例,术后肠粘连5例;确诊阴性12例;多层CT尿路成像检查结果显示,阳性79例,阴性13例;多层CT尿路成像检查诊断非结石性尿路梗阻的灵敏度为97.50%,特异度为96.74%,准确度为96.74%,阳性预测值为98.73%,阴性预测值为84.62%,误诊率为9.09%,漏诊率为15.38%;多层CT尿路成像检查与手术诊断结果的一致性极好(Kappa=0.861,P<0.05)。结论:多层CT尿路成像检查诊断非结石性尿路梗阻的效能极高。Objective:To analyze efficacy of multi-slice CT urography in diagnosis of non-calculous urinary tract obstruction.Methods:A prospective study was conducted on 92 patients with suspected non-calculous urinary tract obstruction admitted to the hospital from January 2019 to June 2021.All patients underwent multi-slice CT urography.Using the results of surgical examination as the gold standard,the efficacy of multislice CT urography in the diagnosis of non-calculous urinary tract obstruction was analyzed.Kappa test was used to test the consistency between multi-slice CT urography and surgery in the diagnosis of non-calculous urinary tract obstruction.Results:The surgical results showed that 80 of the 92 patients with suspected non-calculous urinary tract obstruction were positive,including 28 cases of urologic neoplasms,22 cases of tumor invasion or external compression,20 cases of chronic inflammation of the urinary tract,5 cases of congenital malformations,and 5 cases of postoperative intestinal adhesion;and 12 cases were confirmed negative.The results of multi-slice CT urography showed that 79 cases were positive and 13 cases were negative.The sensitivity,the specificity,the accuracy,the positive predictive value,the negative predictive value,the misdiagnosis rate and the missed diagnosis rate of multi-slice CT urography in the diagnosis of non-calculous urinary tract obstruction were 97.50%,96.74%,96.74%,98.73%,84.62%,9.09% and 15.38%,separately.The consistency between multi-slice CT urography and surgical examination was excellent(Kappa=0.861,P<0.05).Conclusions:Multi-slice CT urography is highly effective in the diagnosis of non-calculous urinary tract obstruction.
