摘 要:目的 评估比较新生儿肺炎运用头罩吸氧与经鼻持续气道正压通气的临床疗效。方法 选取2020年1月—2022年12月于南京医科大学附属江宁医院新生儿科接受新生儿肺炎治疗的94例患儿为研究对象,采用电脑随机法分为对照组(48例)与观察组(46例)。两组均予以新生儿肺炎常规治疗措施,对照组结合头罩吸氧治疗,观察组则结合经鼻持续气道正压通气治疗。对比两组临床相关时间指标、治疗前后血气分析指标、机体指标以及不良反应发生情况。结果 观察组呼吸与心率恢复正常、气促缓解时间、住院用氧总时长均短于对照组,差异有统计学意义(P<0.05);治疗后,观察组血氧分压与血pH值均高于对照组,二氧化碳分压则低于对照组,差异有统计学意义(P<0.05);治疗后,观察组心率以及呼吸频率均低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率(4.35%)低于对照组(18.75%),差异有统计学意义(χ^(2)=4.715,P<0.05)。结论 针对新生儿肺炎予以经鼻持续气道正压通气效果优异,其更有利于优化患儿血气以及机体指标,且治疗手段不良反应甚微,安全有效。Objective To evaluate and compare the clinical effects of head mask oxygen inhalation with nasal continu‐ous positive airway pressure ventilation for neonatal pneumonia.Methods From January 2020 to December 2022,94 children who received neonatal pneumonia treatment in the Department of Neonatology,Jiangning Hospital Affiliated to Nanjing Medical University were selected as the research objects,and were randomly divided into the control group(48 cases)and the observation group(46 cases)by computer.Both groups received routine treatment for neonatal pneu‐monia,while the control group received oxygen therapy with a hood,while the observation group received nasal con‐tinuous positive airway pressure ventilation.Results The observation group had significantly shorter respiratory and heart rate recovery,shortness of breath relief time,and total duration of hospitalization oxygen consumption compared to the control group,the difference was statistically significant(P<0.05).After treatment,the blood oxygen partial pres‐sure and blood pH value in the observation group were higher than those in the control group,while the carbon dioxide partial pressure was lower than that in the control group,the difference was statistically significant(P<0.05).After treatment,the heart rate and respiratory rate in the observation group were lower than those in the control group(4.35%),the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was lower than(18.75%)in the control group,the difference was statistically significant(χ^(2)=4.715,P<0.05).Conclusion Nasal continuous positive airway pressure ventilation for neonatal pneumonia has excellent effect,which is more conducive to optimizing blood gas and body indexes of children,and the treatment has few adverse reactions,which is safe and effective.
