摘　　要：目的 探讨股总动脉内膜中层厚度、超敏C反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)水平与高血压合并阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者严重程度的相关性。方法 收取2021年12月~2022年12月住院连续治疗的高血压患者85例。所有患者均行多导睡眠监测,依据呼吸紊乱(AHI)指数将患者分为高血压合并中重度OSAHS组(A组,n=34)、高血压合并轻度OSAHS组(B组,n=23)和单纯高血压组(C组,n=28)。比较三组间的股总动脉内膜中层厚度,hs-CRP、Hcy水平并分析这三者与高血压合并OSHAS严重程度的相关性。结果 三组间Hcy、hs-CRP水平比较,差异均有统计学意义(P <0.05)。与C组相比较,A、B组Hcy、hsCRP水平显著升高,差异均有统计学意义(P <0.05)。与C组相比较,A、B组股总动脉内膜中层厚度均有升高,A组更为显著,差异均有统计学意义(P <0.05)。相关性分析显示血清Hcy、hs-CRP水平及股动脉内膜中层厚度与AHI、氧减指数(ODI)呈正相关(P <0.05),与最低血氧饱和度水平呈负相关(P <0.05)。结论 高血压合并OSAHS患者股总动脉内膜中层厚度及hs-CRP、Hcy水平均高于单纯高血压患者,且与严重程度呈正相关。股总动脉内膜中层厚度及hs-CRP、Hcy对高血压合并OSAHS患者严重程度有预测评估价值。Objective To investigate the correlation between common femoral artery intima-media thickness,hypersensitive C-reactive protein(hs-CRP),homocysteine(Hcy) levels and the severity of hypertension complicated with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods A total of 85 hypertension patients,who were hospitalized for continuous treatment from December 2021 to December 2022,were collected.All patients underwent polysomnosis monitoring and were divided into hypertension combined with moderate and severe OSAHS group(group A,n = 34),hypertension combined with mild OSAHS group(group B,n = 23) and simple blood pressure group(group C,n = 28) according to AHI index.The common femoral artery intima-media thickness,hs-CRP and Hcy levels were compared among the three groups,and the correlation between the three indexs and the severity of hypertension combined with OSHAS was analyzed.Results There were significant differences in Hcy and hs-CRP levels among the three groups(P<0.05).Compared with group C,the levels of Hcy and hs-CRP in groups A and B were significantly increased,and the differences were statistically significant(P<0.05).Compared with group C,the intimamedia thickness of common femoral artery in groups A and B was increased,and group A was more significant,with statistical significance(P<0.05).Correlation analysis showed that serum Hcy and hs-CRP levels and common femoral artery intima-media thickness were positively correlated with AHI and oxygen reduction index(ODI)(P<0.05),and negatively correlated with minimum blood oxygen saturation(P<0.05).Conclusion The Intima media thickness of common femoral artery,hs-CRP and Hcy in patients with hypertension combined with OSAHS were higher than those in patients with simple hypertension,and were positively correlated with the severity.Intima media thickness of common femoral artery,hs-CRP and Hcy are valuable for detection of patients with hypertension complicated with OSAHS.
