摘 要:目的 采用静息态功能连接(RSFC)和字色Stroop任务观察青少年吸烟成瘾者大脑奖赏回路内部的异常功能连接和认知控制能力,探讨奖赏回路内部异常的功能连接、青少年吸烟成瘾和认知控制之间的关系。方法 搜集20例青少年吸烟成瘾者和19名相匹配的健康对照者的功能磁共振图像,对其大脑奖赏回路进行RSFC分析,得到的异常功能连接与吸烟特征[烟包年(Pack years)和Fagerstr9m尼古丁依赖测试(FTND)]、匹兹堡睡眠质量指数(PSQI),以及Stroop任务表现进行相关性分析。结果 与对照组相比,青少年吸烟成瘾组奖赏回路内部双侧纹状体与左侧前脑岛、双侧丘脑之间的功能连接强度显著增高,右侧纹状体和左侧前脑岛之间的功能连接与Pack years(r=0.487,P=0.030)及PSQI(r=0.501,P=0.024)呈正相关。青少年吸烟成瘾者在字色不一致条件下的反应时间与Pack years(r=0.695,P=0.001)、右侧纹状体和左侧前脑岛之间的RSFC(r=0.496,P=0.026)呈显著正相关。中介效应分析显示,Pack years介导了奖赏回路内部异常的功能连接和Stroop任务表现。结论 青少年吸烟成瘾者大脑奖赏回路内部异常的功能连接、Pack years以及Stroop任务表现(字色不一致条件下的反应时间)之间有着紧密的联系,并且成瘾与睡眠之间也有很多交互,这可能为理解青少年吸烟成瘾的神经机制提供一些新的影像学依据。Objective Using resting-state functional connectivity(RSFC)and word-color Stroop task to observe abnor-mal functional connectivity and cognitive control within the brain reward circuit of adolescent smokers relationship between controls.Methods Functional magnetic resonance images of 20 adolescent smoking addicts and 19 matched healthy con-trols were collected,and their brain reward circuits were subjected to RSFC analysis,resulting in abnormal functional con-nectivity and smoking characteristics(Pack years and FTND).Pittsburgh Sleep Quality Index(PSQI),and Stroop task performance for correlation analysis.Results Compared with the control group,the functional connection strength between the bilateral striatum,the left anterior insula,and the bilateral thalamus in the reward circuits of the adolescent smoking ad-diction group was significantly increased,and the right striatum and left anterior Functional connectivity between the insula was positively correlated with Pack years(r=0.487,P=0.030)and positively correlated with PSQI(r=0.501,P=0.024).Reaction time of adolescent smoking addicts under color-inconsistent conditions versus Pack years(r=0.695,P=0.001),RSFC between right striatum and left anterior insula(r=0.496,P=0.026)were significantly positively correla-ted.Mediation analysis revealed that Pack years mediated abnormal functional connectivity within the reward circuits and Stroop task performance.Conclusion There is a strong relationship between abnormal functional connectivity within the brain reward circuit,Pack years,and Stroop task performance(Incongruent reaction time)in adolescent smokers,and there are also many interactions between smoking addiction and sleep.This study may provide some new imaging evidence for understanding the neural mechanisms of adolescent smoking addiction.
