摘 要:目的:调查癌症患儿营养风险发生率和营养状况。方法:2020年4月至2021年10月,选取四川省某三甲医院儿科361例住院癌症患儿为调查对象,应用儿科营养不良评估筛查工具(STAMP)进行营养风险筛查,采用WHO儿童及青少年生长标准Z值进行营养评估,并分析癌症患儿营养风险、营养状况及两者相关性。结果:所有癌症患儿均存在营养风险,其中高度和中度营养风险分别为25.21%和74.79%。营养不良和营养过剩检出率分别为29.64%和7.48%。营养风险与营养不良有较强相关性(Phi=0.551,P<0.05),且随年龄增加癌症患儿高度营养风险发生率增加(P<0.05)。营养摄入得分在不同营养风险癌症患儿中差异有统计学意义(P<0.05),与营养风险中等强度相关(Cramer’s V=0.342);体格测量得分在不同营养风险和营养状况癌症患儿中差异有统计学意义(P<0.05),与营养风险相关性较强(Cramer’s V=0.911),与营养状况中等强度相关(Cramer’s V=0.449)。结论:癌症患儿营养风险发生率极高,营养不良发生率较高,营养风险与营养不良相关性较强。STAMP能早期识别癌症患儿营养风险,指导临床进行精准且全面的营养管理。Objective:To investigate the incidence of nutritional risk and nutritional status in children with cancer.Methods:From April 2020 to October 2021,a total of 361 children with cancer hospitalized in a 3A hospital in Sichuan Province were selected as the research subjects.The Screening Tool for the Assessment of Malnutrition in Pediatrics(STAMP)was used for nutritional risk screening,and the WHO growth standard Z-value for child and adolescent was used for nutritional assessment.The nutritional risk,nutritional status,and the correlation between them were analyzed for children with cancer.Results:All children with cancer had nutritional risks,with high and moderate nutritional risks of 25.21%and 74.79%,respectively.The detection rates of malnutrition and overnutrition were 29.64%and 7.48%,respectively.There was a strong correlation between nutritional risk and malnutrition(Phi=0.551,P<0.05),and the incidence of high nutritional risk in children with cancer increased with age(P<0.05).There was a statistically significant dfference in nutritional intake scores among children suffering from cancer with different nutritional risks(P<0.05),which was strongly correlated with moderate nutritional risk(Cramer's V=0.342).The difference was statistically significant in physical measurement scores among children suffering from cancer with different nutritional risks and nutritional status(P<0.05),which was strongly correlated with nutritional risk(Cramer's V=0.911),and moderately correlated with nutritional status(Cramer's V=0.449).Conclusion:The incidence of nutritional risk in children with cancer is extremely high,and the incidence of malnutrition is relatively high.There is a strong correlation between nutritional risk and malnutrition.STAMP can early identify the nutritional risks of children with cancer,and guide clinical accurate and comprehensive nutritional management.
