摘　　要：目的:分析黄体功能不全(LPD)所致先兆流产药物保胎治疗后妊娠结局的影响因素。方法:选取2019年10月至2021年12月平顶山市妇幼保健院279例LPD所致先兆流产患者为研究对象,根据药物保胎治疗后妊娠结局分为不良结局组(48例)与结局良好组(231例),采用logistic回归分析LPD所致先兆流产药物保胎治疗后妊娠结局的影响因素。结果:logistic回归分析显示,自然流产、早期阴道出血、宫内大血肿、甲状腺功能减退、尿I-HCG/HCGRP阳性2比例低、治疗前β-HCG<平均值、治疗前孕酮<平均值是LPD所致先兆流产药物保胎治疗后不良妊娠结局的危险因素(P<0.05)。结论:LPD所致先兆流产药物保胎治疗后仍存在流产风险,其妊娠结局受多种因素影响,临床需采取针对性干预措施,以保证患者顺利分娩。Objective:To analyze the influencing factors of pregnancy outcomes in patients with threatened abortion due to luteal phase defect(LPD)after drug therapy for fetal protection.Methods:A total of 279 patients with threatened abortion due to LPD in Pingdingshan Maternal and Child Health Hospital from October 2019 to December 2021 were selected as the research subjects.They were divided into poor outcome group(48 cases)and good outcome group(231 cases)based on the pregnancy outcomes after drug therapy for fetal protection.The influencing factors of pregnancy outcomes in patients with threatened abortion due to LPD after drug therapy for fetal protection were analyzed by logistic regression analysis.Results:Logistic regression analysis showed that spontaneous abortion,early vaginal bleeding,intrauterine hematoma,hypothyroidism,low proportion of urinary I-HCG/HCCRP positive 2,andβ-HCG less than average value before treatment and progesterone less than average value before treatment were risk factors for poor pregnancy outcomes in patients with threatened abortion due to LPD after drug therapy for fetal protection(P<0.05).Conclusion:Patients with threatened abortion due to LPD still have a risk of abortion after drug therapy for fetal protection.The pregnancy outcomes are influenced by multiple factors,and targeted intervention measures need to be taken in clinical practice to ensure a smooth delivery.
