摘　　要：目的 采用ROC曲线模型研究血清人绒毛膜促性腺激素(hCG)、神经轴突导向因子-1(Netrin-1)诊断早期异位妊娠的价值。方法 选择2019年1月—2019年10月嘉兴市妇幼保健院收治的43例异位妊娠患者为研究对象,纳入异位妊娠组;另选择同期于该院检查的正常宫内早孕者40例为正常早孕组。比较两组停经第48天血清Netrin-1、hCG水平,采用ROC曲线分析血清Netrin-1、hCG单一及联合诊断对异位妊娠的早期诊断价值。结果 异位妊娠组Netrin-1、hCG水平均显著低于正常早孕组,差异均有统计学意义(均P<0.05)。经ROC曲线处理,结果显示Netrin-1、hCG对早期异位妊娠均有一定诊断价值,曲线下面积分别为0.730、0.769;Netrin-1、hCG联合诊断早期异位妊娠的特异度为0.775、灵敏度为0.860、准确率0.819、kappa值为0.637。结论 Netrin-1、hCG在早期异位妊娠的诊断中具有重要临床价值。Objective The value of serum Netrin-1 and hCG in the diagnosis of early ectopic pregnancy was studied by receiver operating characteristic curve model.Methods Forty-three patients with ectopic pregnancy confirmed by laparoscopic surgery in Jiaxing Maternal and Child Health Hospital from January 2019 to October 2019 were selected as the research objects and included into the ectopic pregnancy group;forty cases of normal intrauterine early pregnancy examined in our hospital at the same time were selected as the normal early pregnancy group.Serum Netrin-1 and hCG levels were compared between the two groups on the 48th day after menopause.The ROC curve was used to analyze the diagnostic value of single and combined Netrin-1 and hCG in early ectopic pregnancy.Results The levels of Netrin-1 and hCG in ectopic pregnancy group were significantly lower than those in normal early pregnancy group(P<0.05).ROC curve processing showed that Netrin-1 and hCG had certain diagnostic value for early ectopic pregnancy,the area under the curve was 0.730 and 0.769 respectively;the sensitivity,specificity,accuracy,kappa value of Netrin-1 and hCG in the diagnosis of early ectopic pregnancy were 0.860,0.775,0.819 and 0.637 respectively.Conclusion Serum Netrin-1 and hCG have important clinical value in the diagnosis of early ectopic pregnancy.
