苏子降气汤加减联合复方异丙托溴铵对AECOPD患者气流受限及Fas/Apo-1、TNF-α的影响

摘　　要：目的:探究苏子降气汤加减联合复方异丙托溴铵对慢性阻塞性肺疾病急性加重期(AECOPD)患者气流受限及Fas/Apo-1、TNF-α的影响。方法:收集2021年2月—2022年12月我院接收的慢性阻塞性肺疾病急性加重期患者80例,按照随机分配的原则分为对照组和研究组,各40例。对照组进行单一药物治疗(复方异丙托溴铵),研究组则在对照组治疗基础上加以联合苏子降气汤加减治疗。观察并比较两组患者的临床效果,治疗前后气流受限程度指标(FEV1、FVC、FEV1/FVC、FEV1%_(pred))以及T淋巴细胞凋亡相关基因表达水平(Fas/Apo-1、IL-1β、TNF-α)。结果:研究组临床疗效有效率为95.00%,明显较对照组的77.50%高(P<0.05)。治疗后两组气流受限程度指标FEV1、FVC、FEV1/FVC、FEV1%_(pred)水平均较治疗前升高,且治疗后研究组气流受限程度指标水平均较对照组的高(P<0.05)。治疗后两组T淋巴细胞凋亡相关基因指标Fas/Apo-1、IL-1β、TNF-α水平均较治疗前降低,且治疗后研究组T淋巴细胞凋亡相关基因指标水平均较对照组的低(P<0.05)。结论:AECOPD患者经苏子降气汤加减联合复方异丙托溴铵治疗后效果明显,且能在极大程度上提升气流受限程度指标水平,降低T淋巴细胞凋亡相关基因表达水平,值得推广。Objective:To explore the effects of Suzijiangqi Decoction combined with compound ipratropium bromide on airflow restriction,Fas/Apo-1,TNF-αin patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:A total of 80 patients with acute exacerbation of chronic obstructive pulmonary disease admitted to our hospital from February 2021 to December 2022 were randomly divided into control group and study group,each with 40 cases.The control group was treated with single drug(compound ipratropium bromide),and the study group was treated with Suzijiangqi decoction on the basis of the control group.The clinical effects,indexes of airflow limitation(FEV1,FVC,FEV1/FVC,FEV1%_(pred))and expression levels of T lymphocyte apoptosis related genes(Fas/Apo-1,IL-1β,TNF-α)were observed and compared between the two groups.Results:The effective rate of the study group was 95.00%,which was significantly higher than that of the control group(77.50%)(P<0.05).After treatment,the levels of FEV1,FVC,FEV1/FVC and FEV1%_(pred) of the two groups were higher than before treatment,and the levels of the observation group were higher than the control group after treatment(P<0.05).After treatment,the levels of Fas/Apo-1,IL-1βand TNF-αof T lymphocyte apoptosis related gene indexes in both groups were decreased compared with those before treatment,and the levels of T lymphocyte apoptosis related gene indexes in observation group were lower than those in control group after treatment(P<0.05).Conclusion:Suzijiangqi Decoction combined with compound ipratropium bromide has obvious effect in patients with AECOPD,which can greatly improve the index level of airflow restriction degree and reduce the expression level of T lymphocyte apoptosis-related genes,which is worth promoting.
