双上肢垫高体位在复合损伤患者CT扫描检查中的应用研究

摘　　要：目的:探讨双上肢垫高体位在复合损伤患者CT扫描检查中应用效果。方法:将2022年1月—2022年12月我院收治的80例复合损伤患者随机分为A、B、C、D四组,20例一组,A组双上肢在同一水平自然放置在躯体两旁,B组双上肢垫高10°,C组双上肢垫高20°,D组双上肢垫高30°,比较四组辐射剂量(CTDI vol、DLP)、图像质量主观及客观(SD、SNR)评价。结果:四组CTDI vol、DLP值存在差异(P<0.05),从A组到D组CTDI vol、DLP值逐渐减小(P<0.05)。四组颈部椎旁肌肉区、胸锁乳突肌区和胸部左心室区、椎旁肌肉区的SD、SNR值存在差异(P<0.05),从A组到D组颈部椎旁肌肉区、胸锁乳突肌区和胸部左心室区、椎旁肌肉区的SD值逐步减少,SNR值逐步增加(P<0.05)。四组颈部、胸部图像质量主观评分存在差异(P<0.05),从A组到D组颈部、胸部图像质量主观评分逐渐增加(P<0.05)。结论:在确保安全的前提下,双上肢垫高体位可以减少复合损伤患者CT扫描检查中辐射剂量,提高图像质量,且垫高角度越高效果越明显,具有一定临床应用价值。Objective:To investigate the application effect of double upper limb cushion position in CT scanning of patients with compound injury.Methods:A total of 80 patients with compound injury admitted to our hospital from January 2022 to December 2022 were randomly divided into four groups:A,B,C and D,with 20 cases in one group.Group A had two upper limbs naturally placed on both sides of the body at the same level,group B had two upper limb pads 10°,group C had two upper limb pads 20°,and group D had two upper limb pads 30°.The radiation dose(CTDIvol,DLP)and subjective and objective image quality(SD,SNR)of the four groups were compared.Results:The CTDIvol and DLP values of the four groups were different(P<0.05),and the CTDIvol and DLP values decreased gradually from group A to group D(P<0.05).There were differences in the SD and SNR values of the neck paravertebral muscle area,the sternocleidomastoid muscle area,the left ventricular area of the chest and the paravertebral muscle area among the four groups(P<0.05).From group A to group D,the SD values of the neck paravertebral muscle area,the sternocleidomastoid muscle area,the left ventricular area of the chest and the paravertebral muscle area gradually decreased,while the SNR values gradually increased(P<0.05).There were differences in the subjective score of neck and chest image quality among the four groups(P<0.05),and the subjective score of neck and chest image quality gradually increased from group A to group D(P<0.05).Conclusions:On the premise of ensuring safety,double upper limb cushion position can reduce the radiation dose and improve the image quality in CT scan of patients with compound injury,and the higher the Angle of cushion height,the more obvious the effect,which has certain clinical application value.
