摘  要:目的:探讨超声检测主动脉僵硬度指标在冠心病患者临床诊断中的应用价值。方法:80例冠心病患者入选本研究观察组,80名健康人员入选本研究对照组。所有研究对象入院后均采用超声技术对脉搏波传播速度(PWV)、颈总动脉内膜中层厚度(C-IMT)、颈总动脉内径(C-Di)进行检测。比较两组研究对象上述指标测量水平,根据观察指标水平改变情况绘制ROC曲线,根据ROC曲线分析并比较各项检测指标AUC、灵敏度、特异性及准确率。结果:观察组PWV高于对照组,C-IMT、C-Di低于对照组(P<0.05)。PWV、C-IMT、C-Di用于诊断冠心病的最佳截断值分别为:11.79m/s,0.76mm,6.54mm;AUC分别为0.857,0.872,0.853;三项指标用于冠心病的临床诊断均具有良好的诊断效能。联合检测的AUC为0.939,优于三项指标单独检测(P<0.05)。PWV、C-IMT、C-Di联合检测在冠心病诊断中的效能高于各指标单独检测(P<0.05)。结论:冠心病患者患病期间多种主动脉僵硬度指标均会出现明显改变,联合检测PWV、C-IMT、C-Di可为冠心病患者临床早期诊断提供可靠数据支持,同时还有助于后续治疗方案的制定与改善。Objective:To investigate the application value of ultrasonic detection of aortic stiffness in the clinical diagnosis of patients with coronary heart disease.Methods:80 patients with coronary heart disease were included in the observation group and 80 healthy people were included in the control group.Pulse wave velocity(PWV),common carotid artery intima-media thickness(C-IMT)and common carotid artery diameter(C-Di)were measured by ultrasound after admission.The measurement levels of the above indicators were compared between the two groups.ROC curves were drawn according to the changes in the levels of the observed indicators.AUC,sensitivity,specificity and accuracy of each detection indicator were analyzed and compared according to ROC curves.Results:PWV of observation group was higher than control group,while C-IMT and C-Di were lower than control group(P<0.05).The optimal cut-off values of PWV,C-IMT and C-Di for the diagnosis of coronary heart disease were 11.79m/s,0.76mm and 6.54mm,respectively.The AUC values were 0.857,0.872 and 0.853,respectively.All three indexes have good diagnostic efficacy in the clinical diagnosis of coronary heart disease.The AUC of combined detection was 0.939,which was better than that of three independent detection(P<0.05).The efficacy of PWV,C-IMT and C-Di combined detection in the diagnosis of coronary heart disease was higher than that of each index alone(P<0.05).Conclusions:There are significant changes in various aortic stiffness indexes in patients with coronary heart disease during the disease.The combined detection of PWV,C-IMT and C-Di can provide reliable data support for the early clinical diagnosis of patients with coronary heart disease,and also contribute to the formulation and improvement of follow-up treatment plans.
