磁共振表观扩散系数值联合动态增强扫描在局限性胰腺炎半定量影像诊断的应用

摘　　要：目的探讨磁共振表观扩散系数值(eADC值)联合动态增强扫描在局限性胰腺炎诊断中的应用价值。方法选取2020年1月至2022年10月在我院治疗的局限性胰腺炎患者88例和胰腺癌患者90例,同时选取正常胰腺患者80例作为对照组。均行MRI检查,分析各组eADC值和时间信号强度(TIC)曲线类型。结果局限性胰腺炎eADC值为(1.29±0.22)×10^(-3)mm^(2)/s,明显高于胰腺癌(P<0.05),而明显低于正常胰腺(P<0.05);局限性胰腺炎TIC曲线Ⅰ型比例为88.64%,明显高于胰腺癌和正常胰腺(P<0.05);局限性胰腺炎TIC曲线Ⅱ型比例为2.27%,明显低于胰腺癌(P<0.05),与正常胰腺比较,差异无统计学意义(P>0.05);局限性胰腺炎TIC曲线Ⅲ型比例为9.09%,明显低于正常胰腺(P<0.05),与胰腺癌比较,差异无统计学意义(P>0.05);不同性别、年龄、体质量指数、病变部位局限性胰腺炎患者eADC值及TIC曲线类型比较差异无统计学意义(P>0.05);TNM分期Ⅲ+Ⅳ期胰腺癌eADC值为(0.98±0.12)×10^(-3)mm^(2)/s,明显低于Ⅰ+Ⅱ期(P<0.05);eADC值、TIC曲线类型及联合鉴别局限性胰腺炎的ROC曲线下面积分别为0.789、0.843和0.800。结论eADC值和动态增强扫描TIC曲线类型在局限性胰腺炎诊断中有一定应用价值,同时eADC值与胰腺癌TNM分期有关。Objectives To investigate the application value of magnetic resonance apparent diffusion coefficient(eADC)combined with dynamic enhanced scanning in the diagnosis of localized pancreatitis.Methods A total of 88 patients with localized pancreatitis and 90 patients with pancreatic cancer treated in our hospital from January 2020 to October 2022 we selected,and 80 patients with normal pancreas were selected as the control group.All the patients received MRI examination,and theeADC value andtime-signal intensity(TIC)curve type of each group were analyzed.Results The eADC value of localized pancreatitis was(1.29±0.22)×10^(-3)mm^(2)/s,which was significantly higher than that of pancreatic cancer(P<0.05),but significantly lower than that of normal pancreas(P<0.05).The proportion of typeⅠTIC curve in localized pancreatitis was 88.64%,which was significantly higher than that in pancreatic cancer and normal pancreas(P<0.05).The proportion of type II TIC curve in localized pancreatitis was 2.27%,which was significantly lower than that in pancreatic cancer(P<0.05),andthere was no significant difference compared with normal pancreas.The proportion of type III TIC curve in localized pancreatitis was 9.09%,which was significantly lower than that in normal pancreas(P<0.05),andthere was no significant difference compared with pancreatic cancer.There was no significant difference in eADC value and TIC curve type among different gender,age,body mass index,focal pancreatitis patients(P>0.05).The eADC value of TNM stageⅢ+Ⅳpancreatic cancer was(0.98±0.12)×10^(-3)mm^(2)/s,which was significantly lower than that of stage I+II(P<0.05).The area under the ROC curve of eADC value,TIC curve type and combined identification oflocalized pancreatitis were 0.789,0.843 and 0.800,respectively(P<0.05).Conclusions TheeADC value and dynamic enhanced scanning TIC curve type have certain application value in the diagnosis of localized pancreatitis,and the eADC value is related to the TNM stage of pancreatic cancer.
