选择性痔上黏膜切除术与吻合器痔上黏膜环切术对混合痔患者肛肠动力恢复及疼痛度的影响

摘　　要：目的分析选择性痔上黏膜切除术(TST)与吻合器痔上黏膜环切术(PPH)对混合痔患者肛肠动力恢复及疼痛度的影响。方法100例混合痔患者,随机分为对照组及研究组,每组50例。对照组给予PPH治疗,研究组给予TST治疗。比较两组肛肠动力学指标[肛管高压区长度(HPZ)、肛管最大收缩压(MSP)、肛管静息压(ARP)],疼痛程度,并发症发生情况。结果治疗后,研究组HPZ(4.16±0.38)cm长于对照组的(3.69±0.17)cm,MSP(129.83±12.16)mm Hg(1 mm Hg=0.133 kPa)、ARP(43.91±4.16)mm Hg高于对照组的(117.37±9.62)、(38.62±4.05)mm Hg,差异具有统计学意义(P<0.05)。治疗后,研究组视觉模拟评分法(VAS)评分(1.45±0.22)分低于对照组的(3.21±0.62)分,差异具有统计学意义(P<0.05)。研究组并发症发生率4.00%低于对照组的18.00%,差异具有统计学意义(P<0.05)。结论TST比PPH治疗混合痔患者的效果好,可帮助患者恢复肛肠动力,改善患者的疼痛程度,降低并发症。Objective To analyze the effect of tissue selective therapy(TST)and procedure for prolapse and hemorrhoids(PPH)on recovery of anorectal motility and pain level in patients with mixed hemorrhoids.Methods A total of 100 patients with mixed hemorrhoids were randomly divided into control group and study group,with 50 cases in each group.The control group was given PPH treatment,and the study group was given TST treatment.Both groups were compared in terms of anorectal dynamics indexes(high-pressure zone length(HPZ),maximum systolic pressure(MSP),anal resting pressure(ARP)],pain level,and complications.Results After treatment,HPZ of(4.16±0.38)cm in the study group was longer than that of(3.69±0.17)cm in the control group;MSP of(129.83±12.16)mm Hg(1 mm Hg=0.133 kPa)and ARP of(43.91±4.16)mm Hg in the study group were higher than those of(117.37±9.62)and(38.62±4.05)mm Hg in the control group;the differences were statistically significant(P<0.05).After treatment,the visual analogue scale(VAS)score of(1.45±0.22)points in the study group was lower than that of(3.21±0.62)points in the control group,and the difference was statistically significant(P<0.05).The complication rate of the study group was 4.00%,which was lower than that of 18.00%of the control group,and the difference was statistically significant(P<0.05).Conclusion TST is better than PPH in the treatment of mixed hemorrhoids,and can help patients restore anorectal motility,reduce the pain level of patients,and lower complication rates.
