慢性心力衰竭利尿治疗后24 h尿量、尿比重、体重情况及与心功能变化的关系分析

摘　　要：目的分析慢性心力衰竭患者在利尿治疗后24 h尿量、尿比重、体重情况及与心功能变化的关系。方法回顾性分析82例慢性心力衰竭患者临床资料,入院后均开展利尿治疗、护理干预以及健康宣教,依据治疗效果不同将患者分成无效组(5例)、有效组(34例)、显效组(43例)。比较82例患者治疗前后的24 h尿量、尿比重、体重情况,三组患者治疗前后的24 h尿量、尿比重、体重情况。结果治疗后,患者24 h尿量(1341.34±270.88)ml/d、尿比重(1.02±0.01)g/ml均高于治疗前的(923.38±236.51)ml/d、(1.00±0.01)g/ml,体重(58.79±7.04)kg低于治疗前的(64.87±7.39)kg,差异有统计学意义(P<0.05)。治疗后,三组24 h尿量比较,差异有统计学意义(P<0.05)。治疗后,有效组、显效组24 h尿量、尿比重均高于本组治疗前,体重低于本组治疗前,差异有统计学意义(P<0.05)。结论慢性心力衰竭患者利尿治疗后能提高24 h尿量、尿比重,降低体重,进而改善心功能,具有临床应用价值。Objective To analyze the 24 h urine volume,urine specific gravity and body weight and its correlation with changes in cardiac functions after diuretic therapy for chronic heart failure.Methods Clinical data of 82 patients with chronic heart failure were retrospectively analyzed.Diuretic therapy,nursing intervention and health education were carried out after admission,and the patients were divided into ineffective group(5 cases),effective group(34 cases)and significantly effective group(43 cases)according to different therapeutic effects.The 24 h urine volume,urine specific gravity and body weight of 82 patients before and after treatment were compared,as well as the 24 h urine volume,urine specific gravity and body weight of the three groups before and after treatment.Results After treatment,24 h urine volume of(1341.34±270.88)ml/d and urine specific gravity of(1.02±0.01)g/ml in patients were higher than those of(923.38±236.51)ml/d and(1.00±0.01)g/ml before treatment;body weight of(58.79±7.04)kg in patients was lower than that of(64.87±7.39)kg before treatment;the differences were statistically significant(P<0.05).After treatment,the difference of 24 h urine volume among the three groups was statistically significant(P<0.05).After treatment,24 h urine volume and urine specific gravity of patients in effective group and significantly effective group were higher than those before treatment in this group,while body weight was lower than that before treatment in this group.The differences were statistically significant(P<0.05).Conclusion In patients with chronic heart failure,diuretic therapy can improve 24 h urine volume,urine specific gravity,reduce body weight,and thus improve cardiac function,which has clinical application value.
