前列腺电切术麻醉中采用依托咪酯与丙泊酚等容混合液的临床效果及安全性评价

摘　　要：目的探究前列腺电切术麻醉中采取依托咪酯与丙泊酚等容混合液的有效性与安全性。方法76例行前列腺电切术患者为研究对象,依据随机数字表法分为参照组与试验组,各38例。参照组应用丙泊酚麻醉,试验组应用依托咪酯与丙泊酚等容混合液麻醉。对比两组麻醉前及术中血流动力学指标[平均动脉压(MAP)、心率(HR)]、麻醉相关指标及麻醉效果。结果术中,试验组MAP(79.65±3.47)mm Hg(1 mm Hg=0.133 kPa)、HR(82.85±3.58)次/min均高于参照组的(72.18±3.55)mm Hg、(72.76±5.11)次/min,差异有统计学意义(P<0.05)。试验组麻醉起效、苏醒、拔除喉罩时间分别为(5.23±0.53)min、(126.85±13.70)s、(62.66±5.74)s,均短于参照组的(10.01±2.48)min、(416.96±27.63)s、(216.56±12.85)s,差异有统计学意义(P<0.05)。试验组麻醉总有效率94.74%高于参照组的76.32%,差异有统计学意义(P<0.05)。结论依托咪酯与丙泊酚等容混合液麻醉方案的应用,有利于提高前列腺电切术患者的麻醉效果,且血流动力学更加平稳。Objective To investigate the effectiveness and safety of etomidate and propofol mixture in anesthesia for transurethral resection of the prostate.Methods A total of 76 patients undergoing transurethral resection of the prostate were divided into the reference group and the experimental group according to the random numerical table,with 38 cases in each group.The reference group was anesthetized with propofol,and the experimental group was anesthetized with etomidate and propofol mixture.The hemodynamic indexes[mean arterial pressure(MAP),heart rate(HR)]before anesthesia and during surgery,anesthesia-related indexes,and anesthetic effects were compared between the two groups.Results During surgery,MAP of(79.65±3.47)mm Hg(1 mm Hg=0.133 kPa)and HR of(82.85±3.58)beats/min in the experimental group were higher than those of(72.18±3.55)mm Hg and(72.76±5.11)beats/min in the reference group,and the differences were statistically significant(P<0.05).The anesthesia onset time,recovery time and laryngeal mask removal time in the experimental groups were(5.23±0.53)min,(126.85±13.70)s and(62.66±5.74)s,which were shorter than those of(10.01±2.48)min,(416.96±27.63)s and(216.56±12.85)s in the control group,and the differences were statistically significant(P<0.05).The total effective rate of anesthesia of 94.74%in the experimental group was higher than that of 76.32%in the reference group,and the difference was statistically significant(P<0.05).Conclusion The application of etomidate and propofol mixture is beneficial to improve the anesthetic effect of patients undergoing transurethral resection of the prostate,and the hemodynamics are more stable.
