2次早期妊娠发生妊娠瘙痒性荨麻疹样丘疹和斑块患者1例

摘　　要：妊娠瘙痒性荨麻疹样丘疹和斑块(PUPPP)皮损主要为红色风团样丘疹和斑块,另可有丘疱疹、水疱、靶形损害、环形和多环形风团,可融合成片。该病病因不清,主要累及初孕妇,多发生于妊娠末3个月,起病于腹部妊娠纹处。再次妊娠罕见复发,如复发,病情一般较轻。本院该例患者在一胎妊娠8周因“皮肤水肿性红斑、丘疹伴痒1 d”以“妊娠瘙痒性荨麻疹丘疹和斑块”至本科诊治,给予抗炎、抗过敏、降低血管通透性、止痒治疗。患者皮损消退后,至妊娠分娩未再出现类似皮损。二胎妊娠7+4周因“PUPPP”再次至本科治疗,给予上述治疗后,患者皮损再次消退,至第二次妊娠分娩未再出现类似皮损。Pruritic urticarial papules and plaques(PUPPP)of pregnancy are mainly erythematous wheal papules and plaques,but may also have papules,vesicles,target lesions,annular and multi-ring wheals,which may fuse into patches.The etiology is unclear,mainly involving first-time pregnant women,mostly in the last three months of pregnancy,starting at the abdominal stretch marks.Relapse of another pregnancy is rare,if recurrence,the disease is usually mild.The patient in our hospital came to our department for diagnosis and treatment as"pruritic urticarial papules and plaques of pregnancy"at 8 weeks of gestation for"edematous erythema and papules with itching for 1 d",and was given anti-inflammatory,anti-allergic,reducing vascular permeability,anti-itching treatment.After the patient's lesions resolved,no similar lesions appeared until delivery.The patient was treated again for"PUPPP"at 7+4 weeks of gestation in her second pregnancy,and after the above treatment,the patient's skin lesions subsided again.No similar lesions occurred until the second delivery.
