摘 要:目的探讨吸氧方式对心脏瓣膜病患者术后康复的影响。方法选取144例行瓣膜置换术的心脏瓣膜病患者,根据入院时间不同分为对照组(82例)和观察组(62例)。停用呼吸机后,对照组采用鼻导管吸氧法,观察组采用普通面罩吸氧法。比较两组吸氧前后血气指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))及血氧饱和度(SaO_(2))]水平、并发症发生情况、吸氧感受及住院时间。结果吸氧后,两组PaO_(2)、SaO_(2)水平均高于吸氧前,PaCO_(2)水平均低于吸氧前,且观察组PaO_(2)(97.29±1.46)mm Hg(1 mm Hg=0.133 kPa)、SaO_(2)(96.43±2.16)%高于对照组的(94.53±2.84)mm Hg、(93.57±3.13)%,差异有统计学意义(P<0.05);吸氧后,两组PaCO_(2)水平比较,差异无统计学意义(P>0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。观察组患者憋气不适、妨碍交流、影响生活及护理发生率分别为19.35%、90.32%、82.26%,高于对照组的3.66%、0、7.32%,差异有统计学意义(P<0.05)。结论相较于鼻导管吸氧法,普通面罩吸氧法改善心脏瓣膜病患者术后血气指标水平的效果更好,住院时间较短,但患者吸氧感受舒适度较低,临床可根据患者具体情况选用。Objective To discuss the influence of oxygen inhalation method on postoperative rehabilitation of patients with valvular heart disease.Methods A total of 144 valvular heart disease patients undergoing valve replacement were divided into control group(82 cases)and observation group(62 cases)according to different admission time.After stopping the ventilator,the control group used nasal cannula oxygen inhalation,and the observation group used ordinary mask oxygen inhalation.The blood gas index levels before and after oxygen inhalation,complications,oxygen inhalation experience,and length of hospital stay were compared between the two groups.Results After oxygen inhalation,PaO_(2)and SaO_(2)levels in both groups were higher than those before oxygen inhalation in this group,and PaCO_(2)levels were lower than those before oxygen inhalation in this group;PaO_(2)of(97.29±1.46)mm Hg(1 mm Hg=0.133 kPa)and SaO_(2)of(96.43±2.16)%in the observation group were higher than those of(94.53±2.84)mm Hg and(93.57±3.13)%in the control group;the differences were statistically significant(P<0.05).After oxygen inhalation,there was no statistically significant difference in PaCO_(2)level between the two groups(P>0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).The incidence of dyspnea discomfort,obstructing communication,affecting life and nursing in the observation group were 19.35%,90.32%and 82.26%,which were higher than those of 3.66%,0 and 7.32%in the control group,and the differences were statistically significant(P<0.05).Conclusion Compared with nasal cannula oxygen inhalation,ordinary mask oxygen inhalation for postoperative patients with valvular heart disease has a better effect on improving blood gas index levels and a shorter length of hospital stay,but the patient feels less comfortable with oxygen,and the clinical choice can be made based on the patient's specific situation.
