通腑泄热法治疗银屑病血热证疗效观察

摘　　要：目的:观察通腑泄热法治疗银屑病血热证的有效性、安全性。方法:将120例斑块型银屑病(血热证)患者作为研究对象,随机分为治疗组60例和对照组60例。对照组内服凉血解毒汤治疗,治疗组内服加味调胃承气汤联合苦参汤药浴。两组均联合应用窄谱紫外线光疗。比较2组患者治疗前后银屑病皮损面积及严重程度指数(PASI)、治疗总有效率、VAS瘙痒评分、DLQI生活质量评分。结果:治疗组总有效率为94.9%(56/59),对照组为81.0%(47/58),组间比较,治疗组的疗效明显优于对照组,差异有统计学意义(P﹤0.05)。治疗后两组患者的PASI评分和VAS瘙痒评分、DLQI生活质量评分等各项积分均较治疗前明显降低(P﹤0.05),且治疗组治疗后的以上各项积分均明显低于对照组,差异均有统计学意义(P﹤0.05)。结论:加味调胃承气汤联合苦参汤药浴治疗斑块型银屑病疗效确切,能有效修复皮损,减轻瘙痒,改善患者生活质量。Objective:To observe the effectiveness and safety of Tongfu Xiere method on psoriasis with syndrome of blood-heat.Methods:120 cases of patients with plaque psoriasis with syndrome of blood-heat were taken as researched objects,and randomly divided into treatment group and control group,60 cases in each group.The control group was treated with oral Liangxue Jiedu decoction,while the treatment group was treated with oral modified Tiaowei Chengqi decoction and Kushen decoction for medicated bath.In addtion,both groups were treated plus by narrow-band ultraviolet for phototherapy.Psoriasis area and severity index(PASI),total effective rate,VAS pruritus score and DLQI life-quality score of patients were compared between two groups before and after treatment.Results:The total effective rate of treatment group was 94.9%(56/59),and that of control group was 81.0%(47/58);compared between two groups,the clinical efficacy of treatment group was significantly better than that of control group,and the difference was statistically significant(P﹤0.05).After treatment in both groups,various scores of patients such as PASI score,VAS pruritus score and DLQI life-quality score,etc.were significantly lower than before treatment(P﹤0.05),and the above scores of treatment group were significantly lower than those of control group after treatment,and the difference was statistically significant(P﹤0.05).Conclusion:The clinical efficacy of modified Tiaowei Chengqi decoction and Kushen decoction for medicated bath on plaque psoriasis is definite,which can effectively repair skin lesions,relieve pruritus and improve the quality of life of patients.
