摘 要:目的观察电针对糖尿病神经源性膀胱(DNB)大鼠肌球蛋白系统的影响,并探讨其效应机制。方法采用高脂高糖饲料喂养结合单次腹腔注射链脲佐菌素制备DNB大鼠模型,将大鼠分为正常组、模型组、电针组和假电针组,每组10只。电针组电针“肾俞”“膀胱俞”“中髎”“三阴交”,连续8周。检测大鼠糖耐量、尿流动力学及膀胱质量,电镜观察膀胱组织线粒体变化,ELISA检测膀胱组织垂体腺苷酸环化酶激活肽(PACAP)38、环磷酸腺苷(cAMP)和蛋白激酶A(PKA)含量,Western blot检测膀胱组织磷酸化肌球蛋白轻链激酶(p-MLCK)和磷酸化肌球蛋白轻链(p-MLC)表达。结果与正常组比较,模型组大鼠糖耐量降低,膀胱最大容量、膀胱顺应性明显升高(P<0.05),漏尿点压明显降低(P<0.05),膀胱质量明显增加(P<0.05);逼尿肌细胞线粒体肿胀,线粒体嵴断裂或溶解消失;膀胱组织PACAP38、cAMP和PKA含量明显减少(P<0.01),p-MLCK蛋白表达明显降低(P<0.01),p-MLC蛋白表达明显升高(P<0.01)。与模型组比较,电针组大鼠糖耐量差异无统计学意义(P>0.05),膀胱最大容量、膀胱顺应性明显降低(P<0.05),漏尿点压明显升高(P<0.05),膀胱质量明显降低(P<0.05);逼尿肌细胞部分线粒体轻度肿胀;膀胱组织PACAP38、cAMP和PKA含量明显增加(P<0.01),p-MLCK蛋白表达明显升高(P<0.05),p-MLC蛋白表达明显降低(P<0.05)。结论电针可通过激活DNB模型大鼠膀胱PACAP/cAMP/PKA信号通路,调控其肌球蛋白系统相关收缩元件磷酸化水平,修复逼尿肌细胞受损的线粒体,调节能量代谢,改善膀胱肥大,调控膀胱内压和顺应性,修复DNB模型大鼠受损的膀胱功能。Objective To observe the effects of electroacupuncture on diabetic neurogenic bladder(DNB)rat model;To explore the mechanism.Methods DNB rat model was prepared by feeding high fat and sugar diet and intraperitoneal injection of streptozotocin,and the rats were divided into normal group,model group,electroacupuncture group and sham-electroacupuncture group,with 10 rats in each group.After 8 weeks of electroacupuncture intervention at“Shenshu”“Pangguangshu”“Zhongliao”and“Sanyinjiao”acupoints,glucose tolerance,urodynamics and bladder quality were detected,the changes of mitochondria in bladder tissue were observed by transmission electron microscopy,the expressions of PACAP38,cAMP and PKA in bladder tissue were detected by ELISA,the expression of p-MLCK and p-MLC protein in bladder tissue were detected by Western blot.Results Compared with the normal group,glucose tolerance of the rats in the model group decreased,the bladder maximum capacity and bladder compliance increased significantly(P<0.05),the leakage point pressure decreased significantly(P<0.05),and the bladder quality increased significantly(P<0.05);the mitochondria of detrusor cells were swollen,and the cristae of mitochondria were broken or dissolved;the contents of PACAP38,cAMP and PKA in bladder tissue decreased significantly(P<0.01),the expression of p-MLCK protein decreased significantly(P<0.01),and the expression of p-MLC protein increased significantly(P<0.01).Compared with the model group,there was no significant difference in glucose tolerance of the rats in electroacupuncture group(P>0.05),the maximum bladder capacity and bladder compliance were significantly reduced(P<0.05),the urine leakage point pressure significantly increased(P<0.05),and the bladder quality was significantly reduced(P<0.05);some mitochondria of detrusor cells were slightly swollen;the contents of PACAP38,cAMP and PKA in bladder tissue significantly decreased(P<0.01),the expression of p-MLCK protein significantly increased(P<0.05),and the expression of p-MLC
