柴胡疏肝汤加减治疗高血压病伴焦虑疗效观察

摘　　要：目的探讨柴胡疏肝汤加减治疗高血压病伴焦虑的临床疗效。方法按照随机数字表法将60例高血压病伴焦虑症患者分成对照组和观察组。2组均采用常规降压治疗,对照组另外联合草酸艾司西肽普兰片治疗,观察组另外联合柴胡疏肝汤加减治疗。观察2组治疗前后中医证候积分(胸部刺痛、胸胁胀痛、抑郁或易怒急躁、腹胀)、血压水平(SBP、DBP)、焦虑自评量表(SAS)、生活质量评分、单胺神经递质[多巴胺(DA)、去甲肾上腺素(NE)、5-羟色胺(5-HT)]水平变化,并比较2组治疗总有效率及安全性。结果观察组治疗后各项中医证候积分、SBP、DBP、SAS评分均低于对照组(P<0.05),生活质量评分、DA、NE、5-HT水平及治疗总有效率均高于对照组(P<0.05);2组均无严重不良反应发生。结论柴胡疏肝汤加减结合常规降压措施治疗高血压伴焦虑疗效确切,安全有效,能够有效控制血压水平,调节单胺神经递质水平,缓解焦虑症状。Objective To explore the clinical effect of modified Chaihu Shugan Decoction in the treatment of hypertension with anxiety.Methods According to the method of random number table,60 patients with hypertension and anxiety were divided into a control group and an observation group.The two groups were treated with a conventional antihypertensive therapy,and the control group was additionally treated with escitalopram oxalate tablets,while the observation group was additionally treated with modified Chaihu Shugan Decoction.The changes of the TCM syndrome scores(chest pain,chest and flank pain,depression or irritability,abdominal distension),the blood pressure levels(SBP,DBP),scores of the self-rating anxiety scale(SAS),quality of life scores,monoamine neurotransmitter levels[dopamine(DA),norepinephrine(NE),5-hydroxytryptamine(5-HT)]were observed in the two groups before and after treatment.The total effective rate and safety of the two groups were compared.Results After treatment,the scores of the TCM syndromes,SBP,DBP and SAS in the observation group were lower than those in the control group(P<0.05),while the quality of life score,the levels of DA,NE,5-HT and the total effective rate were higher than those of the control group(P<0.05).There were no serious adverse reactions in both groups.Conclusion The combination of modified Chaihu Shugan Decoction and conventional antihypertensive measures is effective and safe in the treatment of hypertension with anxiety,which can effectively control the blood pressure levels,regulate the level of monoamine neurotransmitter and relieve the anxiety symptoms.
