清润肃肺方治疗Ⅱ型呼吸衰竭阴虚痰热证伴便秘的效果

摘　　要：目的探讨清润肃肺方治疗Ⅱ型呼吸衰竭阴虚痰热证伴便秘患者的临床效果。方法选取2020年6月至2022年5月宿迁市中医院收治的100例Ⅱ型呼吸衰竭患者作为研究对象,按照随机数字表法将其分为治疗组(50例)与对照组(50例),两组均予西医基础治疗,治疗组采用清润肃肺方改善便秘,对照组予乳果糖通便。比较两组的临床疗效、治疗前后的便秘症状积分、血气分析结果及住院天数。结果治疗组的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗组治疗后的排便时间、排便频率、排便困难、粪便性状、下坠不尽胀感、腹胀症状积分低于对照组,差异有统计学意义(P<0.05)。治疗后,治疗组的二氧化碳分压(PaCO_(2))低于对照组,差异有统计学意义(P<0.05),两组患者的pH、血氧分压(PaO_(2))比较,差异无统计学意义(P>0.05)。治疗组的住院天数少于对照组,差异有统计学意义(P<0.05)。结论清润肃肺方治疗Ⅱ型呼吸衰竭阴虚痰热证伴便秘患者,能明显改善患者便秘症状,降低PaCO_(2),减少住院天数,疗效明显。Objective To explore the clinical efficacy of Qingrun Sufei Decoction in the treatment of typeⅡrespiratory failure patients with yin deficiency and phlegm-heat syndrome with constipation.Methods A total of 100 patients with typeⅡrespiratory failure admitted to Suqian Traditional Chinese Medicine Hospital from June 2020 to May 2022 were selected as the study objects,and were divided into treatment group(50 cases)and control group(50 cases)according to random number table method.Both groups were treated with basic western medicine.The treatment group was treated with Qingrun Sufei Decoction to improve constipation,and the control group was treated with Lactulose to relieve constipation.The clinical efficacy,constipation symptom score before and after treatment,blood gas analysis results and hospitalization days were compared between the two groups.Results The total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of defecation time,frequency of defecation,difficulty of defecation,fecal character,feeling of tumescence and abdominal distension in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the partial pressure of carbon dioxide(PaCO_(2))in the treatment group was lower than that in the control group,the difference was statistically significant(P<0.05),while the pH and blood oxygen partial pressure(PaO_(2))between the two groups were not statistically significant(P>0.05).The hospitalization days of the treatment group was less than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Qingrun Sufei Decoction can significantly improve constipation symptoms,reduce PaCO_(2),and reduce the number of days in hospital in patients with typeⅡrespiratory failure due to yin deficiency and phlegm-heat syndrome.
