摘 要:目的比较磺胺嘧啶银(SSD)单独及联合水蛭素治疗深Ⅱ度烧伤大鼠的效果。方法取30只SD大鼠,通过局部皮肤热水浴建立深Ⅱ度烧伤模型。将25只造模成功的大鼠随机分为A组(8只)、B组(9只)、C组(8只),给予A组大鼠凡士林涂抹创面,给予B组大鼠SSD乳膏涂抹创面,给予C组大鼠SSD乳膏涂抹创面及局部注射水蛭素。观察各组大鼠的一般情况及创面愈合情况,计算创面愈合率,并比较各组大鼠创面愈合时间。结果A组和B组的部分大鼠一般情况较差,有肺部感染征象,且出现死亡的情况;C组大鼠的整体状况较佳,仅个别大鼠存在轻微的感染症状,且无大鼠死亡。A组和B组大鼠创面基底部可见较多淤血,微血栓较多,而C组大鼠创面的微血栓较少。3组大鼠的创面愈合时间差异无统计学意义(P>0.05),但在致伤后第19~20天A组和C组所有大鼠创面均愈合,而在致伤后第21天B组仍有大鼠的创面仍未愈合。与B组相比,A组大鼠在致伤后第10天、第18天和第19天的愈合率更高,C组大鼠在致伤后第10天、第13天、第18天和第19天的愈合率更高(均P<0.05)。结论单纯使用SSD会导致深Ⅱ度烧伤大鼠模型的创面延迟愈合,而SSD联合水蛭素可以通过改善微循环来促进深Ⅱ度烧伤大鼠的创面愈合,且可降低感染风险。Objective To compare the effect of silver sulfadiazine(SSD)alone and combined with hirudin for the treatment of rats with deepⅡ-degree burn.Methods A total of 30 SD rats were obtained,and the deep Ⅱ-degree burn model was established through hot water bath of local skin.The 25 rats for successful modeling were randomly assigned to groups A(8 rats),B(9 rats),and C(8 rats).Group A received vaseline smearing to wound,group B received SSD emulsifiable paste smearing to wound,and group C received SSD emulsifiable paste smearing to wound and local injection of hirudin.The general status and wound healing status of rats in various groups were observed,the wound healing rate was calculated,and the wound healing time of rats was compared between various groups.Results In groups A and B,some rats presented as poor general status and symptom of pulmonary infection,and death was observed.In group C,rats presented as favorable overall status,mild symptoms of infection was only found in some rats,and no death occurred.There were a relatively large amount of congestion in the base of wound,and a relatively large amount of microthrombus in groups A and B,whereas a small amount of microthrombus on wound in group C.No statistically significant difference in the wound healing time was found between the 3 groups(P>0.05).However,on the 19 th to 20 th day after trauma induction,all rats′wounds in groups A and C were healed,while on the 21 st day after trauma induction,wounds of some rats in group B were still unhealed.Compared with group B,the healing rate of rats in group A was higher on the 10 th,18 th,and 19 th day after trauma induction,and the healing rate of rats in group C was higher on the 10 th,13 th,18 th,and 19 th day after trauma induction(all P<0.05).Conclusion Employing SSD alone will cause delayed wound healing in rats with deep Ⅱ-degree burn,whereas SSD combined with hirudin can promote wound healing in rats with deepⅡ-degree burn,and decrease infectious risk through ameliorating micro-regulation.
