突发新冠疫情下的中小医院PCR实验室实践探讨

摘　　要：目的:为了总结新型冠状病毒肺炎(COVID-19)突发疫情下,医院PCR实验室应对区域全员核酸检测的实践经验。方法:在苏州“2·13”突发新冠肺炎疫情事件中,笔者所在医院PCR实验室历经多轮区域全员核酸检测,PCR实验室在长达24天的高负荷运转中,遇到的各种突出问题,并及时做出优化及解决方案,使实验室正常安全的高效运转,其中包括工作流程优化、工作人员安排、生物安全防护、仪器设备维护、心理健康疏导等方面。结果:PCR实验室22名工作人员零感染;24天内PCR实验室检测咽/鼻拭子标本67891管,共计308646人份,单日最大检测管数为6693管,单日最大检测人数为34264人;共检出阳性标本9例,其中隔离点标本8例(成人6例,儿童2例),其中本院发热门诊标本1例(儿童);在工作中遇到并优化解决突出问题18项。结论:积累了应对区域全员核酸筛查的宝贵工作经验,保质保量地完成了核酸检测任务,为此次突发新冠疫情做出了积极的贡献。Objective:To summarize the practical experience of PCR laboratory in hospitals under the sudden outbreak of COVID-19,how to deal with the nucleic acid detection of all personnel in the region.Methods:During the"2.13"outbreak of new coronary pneumonia in Suzhou,the PCR laboratory of the author's hospital has undergone multiple rounds of nucleic acid testing for all staffin the region.During the 24-day high-load operation,the PCR laboratory encountered various outstanding problems,and make optimizations and solutions in a timely manner to make the laboratory operate normally,safely and efficiently,including workflow optimization,staff arrangement,biosafety protection,equipment maintenance,mental health counseling,etc.Results:22 staffin the PCR laboratory had zero infection;within 24 daysthe PCR laboratory tested 67891 tubes of throat/nasal swab specimens,with a total of 308,646 samples,the maximum number of tubes tested in a single day was 6,693,and the maximum number of people tested in a single day was 34,264;a total of 9 positives were detected,including 8 specimens at the isolation point(6 adults,2 children),and 1 specimen(children)from the fever clinic in our hospital;Encountered and optimized 18 outstanding problems at work.Conclusion:We have accumulated valuable working experience in responding to nucleic acid screening of all employees in the region,completed the task of nucleic acid detection with quality and quantity,and made positive contributions to the outbreak of the new crown epidemic.
