基于数据挖掘技术的中药内服治疗成人桡骨远端骨折用药规律分析

摘　　要：[目的]应用数据挖掘技术,剖析中药内服治疗成人桡骨远端骨折的用药规律,为临床应用提供参考。[方法]检索中国知网、维普数据库、万方数据库、中国生物医学文献、Pubmed、Embase、Cochrane Library数据库中自建库至2022年11月公开发表的闭合复位或切开复位术后联合中药内服治疗成人桡骨远端骨折的中英文文献,筛选并提取中药方剂,应用Microsoft Excel 2016建立数据库,进行药物频数及性味归经统计分析;应用IBM SPSS Modeler 18.0软件、R 4.2.0软件和Cytoscape 3.9.1软件进行关联规则分析、组合集合可视化分析、聚类分析以及网络链接关系分析。[结果]共筛选出符合纳入标准文献127篇,有效中药方剂200首,中药153味,其中当归、红花、续断、川芎、赤芍、骨碎补等药物频数较高,药性以温性居多,药味以甘味、苦味、辛味居多,归经以肝、脾、心、肾经为主。通过关联规则分析得到关联度较高药物组合19项(对药5项,角药14项),高置信度关联规则有“乳香→没药”“川芎-红花→当归”等。通过聚类分析获得4个聚类组合,均为多味药物聚合组。通过网络拓扑分析可得核心药物为当归、红花、川芎、续断、骨碎补、赤芍。[结论]中药内服治疗成人桡骨远端骨折用药以甘、苦、辛味为主,温性药居多,归肝、脾、心、肾经药物配伍使用,多用补血、活血、化瘀、续筋接骨之法,对于中医临床治疗成人桡骨远端骨折有一定借鉴意义。[Objective]Application of data mining technology,analysis of traditional Chinese medicine oral treatment of distal radius fractures in adult medication rules,provide a reference for clinical application.[Methods]Chinese and English literatures on the treatment of adult distal radius fractures after closed reduction or open reduction combined with oral administration of traditional Chinese medicine were retrieved from the databases of CNKI,VIP,Wanfang,Chinese biomedical literature,Pubmed,Embase and Cochrane Library from the establishment of the database to November 2022.The prescriptions of traditional Chinese medicine were screened and extracted.Microsoft Excel 2016 was used to establish a database for statistical analysis of drug frequency and flavor tropism.IBM SPSS Modeler 18.0 software,R 4.2.0 software and Cytoscape 3.9.1 software were used for association rule analysis,Visual analysis of combination set,cluster analysis and network link relationship analysis.[Results]A total of 127 literatures that met the inclusion criteria were screened out,including 200 effective Chinese medicine prescriptions and 153 flavors of Chinese medicine.Among them,the frequency of angelica,safflower,dipsacus,chuanxiong,red peony root,drynaria,et al.drugs are higher,and the medicinal properties are mostly warm.The flavors are mostly sweet,bitter and pungent,and the meridians are mainly liver,spleen,heart and kidney.Through the analysis of association rules,19 drug combinations with high correlation degree(5 pairs of drugs and 14 angle drugs)were obtained.The high confidence association rules were“frankincense→myrrh”“Chuanxiong-safflower→angelica”and so on.Four cluster combinations were obtained by cluster analysis,all of which were multi-flavor drugs polymerization groups.Through network topology analysis,the core drugs were angelica,safflower,Chuanxiong,dipsacus,rhizoma drynariae and radix paeoniae rubra.[Conclusion]Oral administration of traditional Chinese medicine for the treatment of adult distal radius fractures
