摘  要:目的探讨信号肽-CUB-表皮生长因子结构域包含蛋白1(SCUBE1)与ST段抬高型心肌梗死(STEMI)患者炎性反应、血栓负荷及PCI术后主要不良心血管事件(MACE)的相关性。方法选取武汉科技大学医学院附属华润武钢总医院心血管内科收治的STEMI患者347例,根据PCI术后血管造影血栓负荷分为低血栓负荷组140例和高血栓负荷组207例。采用ELISA测定血清SCUBE1水平。结果高血栓负荷组血清SCUBE1基线水平显著高于低血栓负荷组(P<0.01)。各时间点SCUBE1和白细胞介素6呈正相关(P<0.01)。基线血清SCUBE1与基线肌钙蛋白I、B型钠尿肽呈正相关(P<0.05)。血清SCUBE1水平诊断高血栓负荷及MACE的ROC曲线下面积分别为0.750(95%CI:0.699~0.802)、0.770(95%CI:0.709~0.831)。血清SCUBE1是PCI术后MACE发生的独立危险因素(P<0.01)。Kaplan Meier分析显示,血清SCUBE1低水平患者MACE预后优于高水平患者(P<0.01)。结论血清SCUBE1水平升高与STEMI患者高血栓负荷、炎性反应及预后不良密切相关。SCUBE1有望成为STEMI患者危险分层的生物标志物。Objective To investigate the correlation of signal peptide-CUB-EGF domain-containing protein 1(SCUBE1)with inflammatory response,thrombus burden and MACE in patients with STEMI after undergoing PCI.Methods A total of 347 STEMI patients admitted to our department were recruited.According to their angiographic thrombus burden after PCI,they were divided into a low thrombus burden group of 140 cases and a high thrombus burden group of 207 cases.Serum SCUBE1 level was determined by ELISA.Results The baseline level of serum SCUBE1 was significantly higher in the high burden group than the low burden group(P<0.01).SCUBE1 level was positively correlated with IL-6 level at each time point(P<0.01).The baseline serum SCUBE1 level was positively correlated with cTnI and brain natriuretic peptide levels(P<0.05).The areas under ROC curve of serum SCUBE1 level for high thrombus burden and MACE were 0.750(95%CI:0.699-0.802)and 0.770(95%CI:0.709-0.831),respectively.Serum SCUBE1 was an independent risk factor for MACE after PCI(P<0.01).Kaplan-Meier analysis showed that the patients with low serum SCUBE1 level had better MACE prognosis than those with high level(P<0.01).Conclusion Increased serum SCUBE1 level is closely associated with high thrombus burden,inflammatory response,and poor prognosis in STEMI patients.SCUBE1 is expected to be a biomarker for risk stratification for STEMI patients.
