腹水降钙素原、白细胞介素-6在肝硬化腹水合并自发性腹膜炎中的检验分析

摘　　要：目的分析腹水降钙素原(procalcitonin,PCT)、白细胞介素-6(interleukin-6,IL-6)在肝硬化腹水合并自发性腹膜炎(spontaneous peritonitis,SP)中的检验水平。方法选择三明市第一医院2020年9月—2021年9月收治肝硬化腹水合并SP患者50例为观察组,同时纳入同一时期的47例单纯肝硬化腹水患者为对照组。两组患者均采用腹水PCT与IL-6进行检测分析,使用化学发光免疫法测定患者腹水PCT、IL-6水平,分析PCT联合IL-6的诊断价值。结果观察组患者腹水PCT与IL-6水平均高于对照组,差异有统计学意义(P<0.05);以肝硬化腹水合并SP为阳性,以单纯肝硬化腹水为阴性;腹水PCT检出肝硬化腹水合并SP患者40例,其中真阳性患者35例;IL-6检出肝硬化腹水合并SP阳性38例,其中真阳性患者32例;腹水PCT联合IL-6检测肝硬化腹水合并SP阳性50例,真阳性49例;腹水PCT联合IL-6检测特异度、敏感度以及准确度均高于95.00%。结论肝硬化腹水合并SP患者诊断中,患者腹水PCT、IL-6水平显著高于单纯肝硬化腹水患者,腹水PCT联合IL-6检测综合应用价值高,建议临床推广使用。Objective To analyze the level of procalcitonin(PCT and interleukin-6(IL-6) in ascites of cirrhosis with spontaneous peritonitis.Methods A total of 50 patients with cirrhosis ascites combined with SP admitted to Sanming First Hospital from September 2020 to September 2021 were selected as the observation group,and 47 patients with simple cirrhosis ascites in the same period were included as the control group.Ascites PCT and IL-6 were detected and analyzed in both groups,and the levels of PCT and IL-6 in ascites were measured by chemiluminescence immunoassay,and the diagnostic value of PCT combined with IL-6 was analyzed.Results The levels of PCT and IL-6 in ascites of patients in the observation group were higher than those in the control group,and the difference between the groups was statistically significant(P<0.05).Positive for ascites due to cirrhosis combined with spontaneous peritonitis,negative for ascites due to simple cirrhosis.PCT in ascites was detected in 40 patients with cirrhosis ascites complicated with spontaneous peritonitis,of which 3 5 were true positive.IL-6 detected 38 patients with cirrhosis ascites and spontaneous peritonitis,of which 32 were truly positive.Ascites PCT combined with IL-6were detected in 50 patients with cirrhosis ascites combined with spontaneous peritonitis,and 49 were true positive.The specificity,sensitivity and accuracy of ascites PCT combined with IL-6 were higher than 95.00%.Conclusion In the diagnosis of cirrhotic ascites complicated with SP,the levels of PCT and IL-6 in ascites are significantly higher than simple cirrhosis ascites.The combined application value of ascites PCT and IL-6 detection is high,and it is recommended to promote clinical use.
