经鼻内镜手术与显微镜手术治疗78例垂体瘤患者对比研究

摘　　要：目的:对比经鼻内镜手术与显微镜手术治疗垂体瘤患者的效果。方法:回顾性收集2022年1月~2022年12月本院78例垂体瘤患者病例资料,按手术方案不同分成观察组(n=39)、对照组(n=39)。对照组接受显微镜手术治疗,观察组接受鼻内镜手术治疗。对比两组手术指标、肿瘤完整切除率、手术前后视觉功能、嗅觉功能、激素指标水平、并发症发生率。结果:观察组住院耗时较对照组短,术中失血量较对照组少,手术耗时较对照组长,肿瘤完整切除率较对照组高,并发症发生率较对照组低(P<0.05);术后7d观察组视野平均缺损、模式标准差、嗅觉功能评分较对照组低,视野指数较对照组高(P<0.05);术后7d观察组血清催乳素、生长激素、促肾上腺皮质激素含量均较对照组低(P<0.05)。结论:与显微镜手术治疗垂体瘤患者相比,经鼻内镜手术治疗于提高完整切除率、优化手术指标、改善视觉功能及激素水平、减少并发症方面更具优势,且对嗅觉功能影响更小,但会略微延长手术耗时。Objective:To compare the efficacy of transnasal endoscopic surgery and microscopic surgery in the treatment of pituitary tumor(PA).Methods:The data of 78 patients with PA in our hospital(from January 2022 to December 2022)were retrospectively collected and divided into observation group(n=39)and control group(n=39)according to different surgical protocols.The control group received microscopic surgery,and the observation group received nasal endoscopic surgery.The surgical indexes,tumor complete resection rate,visual function,olfactory function,hormone indexes and complication rate were compared between the two groups.Results:The duration of hospitalization in the observation group was shorter than that in the control group,the intraoperative blood loss was less,the operation time was longer than that in the control group,the tumor complete resection rate was higher,the complication rate was lower(P<0.05).The score of MD,PSD and olfactory function in the observation group was lower than that in the control group 7 days after surgery,while the VFI in the observation group was higher than that in the control group(P<0.05).The contents of serum PRL,GH and ACTH in the observation group were lower than those in the control group 7 days after surgery(P<0.05).Conclusion:Compared with microscopic surgery for PA patients,endoscopic nasal surgery has more advantages in improving complete resection rate,optimizing surgical indicators,improving visual function and hormone levels,and reducing complications.It also has less effect on olfactory function,but slightly prolongs surgical time.
