对比经尿道前列腺等离子体双极电切术和经尿道钬激光前列腺剜除术治疗前列腺增生的应用价值

摘　　要：目的:探讨经尿道前列腺等离子体双极电切术和经尿道钬激光前列腺剜除术治疗前列腺增生的应用价值。方法:遴选2019年1月~2022年12月在本院手术治疗的60例前列腺增生的患者为观察对象,按照随机数字表法将其分为对照组和观察组,每组患者各30例。其中,对照组患者采用经尿道前列腺等离子体双极电切术治疗,观察组则给予经尿道钬激光前列腺剜除术治疗,对比分析两组患者手术情况、手术前后最大尿流率、平均尿流率、残余尿量、血清炎性因子水平及并发症发生情况。结果:(1)与对照组相比,观察组患者手术时间、拔管时间及住院时间更短、术中出血量更少,且组间数据比较差异明显(P<0.05);(2)两组患者手术治疗前最大尿流率、平均尿流率、残余尿量比较差异不明显(P>0.05),但手术治疗后观察组患者最大尿流率、平均尿流率多于对照组,而残余尿量则少于对照组,且组间数据比较差异明显(P<0.05);(3)两组患者治疗血清炎性因子各指标水平比较差异不明显,但治疗后观察组患者前列腺素E2、肿瘤坏死因子-α及白细胞介素-6水平均低于对照组,且组间数据比较差异明显(P<0.05);(4)观察组患者并发症总发生率则低于对照组,且组间数据比较差异明显(P<0.05)。结论:经尿道前列腺等离子体双极电切术和经尿道钬激光前列腺剜除术治疗前列腺增生的应用价值相当,但经尿道钬激光前列腺剜除术治疗手术时间短、出血量少、并发症少、恢复快等优势。Objective:To investigate the clinical value of plasma resection of prostate and holmium laser enucleation in the treatment of benign prostatic hyperplasia.Methods:A total of 60 patients with benign prostatic hyperplasia who underwent surgical treatment in our hospital from January 2019 to December 2022 were selected as subjects.They were divided into a reference group and a study group according to a random number table method,with 30 patients in each group.Among them,patients in the reference group were treated with plasma resection of the prostate,while patients in the research group were treated with holmium laser enucleation.The surgical conditions,maximum preoperative and postoperative urinary flow rate,average urinary flow rate,residual urine volume,and serum inflammatory factor levels and complications.Results:①Compared with the reference group,the study group had shorter surgical time,extubation time,and hospital stay,and less intraoperative bleeding,with significant differences in data between the groups(P<0.05);②There was no significant difference in the maximum urine flow rate,average urine flow rate,and residual urine volume between the two groups before surgical treatment(P>0.05),but after surgical treatment,the maximum urine flow rate,average urine flow rate,and residual urine volume in the study group were more than those in the reference group,while the residual urine volume was less than those in the reference group,and there was a significant difference in data between the groups(P<0.05);③There was no significant difference in the levels of serum inflammatory factors between the two groups,but after treatment,prostaglandin E2 and tumor necrosis factor-αand interleukin 6 in the observation were lower than those in the control group,and the data between the groups were significantly different(P<0.05);④The total incidence of complications in the study group was lower than that in the reference group,and there was a significant difference in data between the groups(P<0.05).Conclusion:Plasma
