摘  要:目的:探讨3.0 T磁敏感加权成像在腔隙性脑梗塞中微出血的应用价值。方法:选取2020年12月~2022年1月收治的40例腔隙性脑梗塞患者纳入实验组,另选取同时期体检中心的无症状体检者40例纳入对照组。两组均行颅脑常规磁共振(包括T2WI、T2FLAIR、T1FLAIR)及磁敏感加权成像(SWI)、磁共振弥散加权成像(DWI)检查。对比不同成像方法上的微出血灶数量、部位、阳性率等数据。结果:在实验组中,常规MRI扫描共发现5例患者出现斑点状的偏长T2WI信号,在T2-FLAIR上表现为稍高或稍低信号,3例患者出现斑点状的偏长T1WI信号。有7例出现DWI明显高信号。对照组中,有1例发生点状偏长T2WI信号,在T2-FLAIR上表现为稍高信号,1例患者出现点状的等长T1WI信号。有2例出现DWI明显高信号。CMBs并在SWI序列中表现为斑点状低信号,边缘锐利且光滑。实验组患者在SWI序列中可见微出血灶共计67个,分为轻度13例,中度7例,重度2例。对照组在SWI序列中可见微出血灶共计6个,均为轻度。SWI显示CMBs的阳性率显著高于DWI/T1WI/T2WI(P<0.05)。结论:与常规序列相比,SWI序列对腔隙性脑梗塞患者微出血灶的显示价值更佳。对于临床上确诊为腔隙性脑梗塞的患者进行早期SWI序列扫描,对判断微出血灶的部位、数量具有一定价值。如果条件允许,可考虑将SWI序列扫描纳入首选检查。Objective:To explore the application value of 3.0T magnetic sensitivity weighted imaging in microbleeding in lacunar cerebral infarction.Methods:40 patients with lacunar cerebral infarction treated from December 2020 to January 2022 were included in the experimental group,and 40 healthy subjects from the physical examination center in the same period were included in the control group.Both groups underwent routine magnetic resonance imaging(including T2WI,T2FLAIR,T1FLAIR),magnetic sensitivity weighted imaging(SWI)and magnetic resonance diffusion weighted imaging(DWI).The number,location and positive rate of microbleeding foci on different imaging methods were compared.Results:In the experimental group,conventional MRI scanning found that 5 patients had spotted long T2WI signals,which showed slightly higher or lower signals on T2-FLAIR,and 3 patients had spotted long T1WI signals.DWI was significantly high in 7 cases.In the control group,one patient had punctate long T2WI signal,which showed slightly high signal on T2-FLAIR,and one patient had punctate equal length T1WI signal.DWI was significantly high in 2 cases.CMBS also showed spotted low signal in SWI sequence,with sharp and smooth edges.In the experimental group,there were 67 microbleeding foci in SWI sequence,which were divided into 13 cases of mild,7 cases of moderate and 2 cases of severe.In the control group,there were 6 microbleeding foci in SWI sequence,all of which were mild.SWI showed that the positive rate of CMBS was significantly higher than that of DWI/T1WI/T2WI(P<0.05).Conclusions:Compared with conventional sequence,SWI sequence has more value in displaying microbleeds in patients with lacunar cerebral infarction.For patients with lacunar cerebral infarction,early SWI sequence scanning is of certain value in judging the location and quantity of microbleeding foci.If conditions permit,SWI sequence scanning can be considered as the preferred examination.
