摘 要:目的:探讨荧光RT-PCR仪与测序仪在丙型肝炎病毒(HCV)基因型检测中的应用,为临床诊断提供依据。方法:收集2018年1月~2020年2月86例明确诊断为丙型病毒性肝炎患者的外周静脉血2mL,分别采用荧光RTPCR仪和测序仪对血清进行HCV基因分型测定。结果:荧光RT-PCR共分型84例丙型病毒性肝炎患者血清,其中基因1型56例(66.7%),非基因1型28例(33.3%),2例分型不确定。测序仪共分型82例,其中基因1型59例(70.2%),均为基因1b型,非基因1型23例(27.3%),包括基因2a型9例、3b型1例、4a型2例、4k型2例、6a型9例,4例分型不确定。以测序结果为“金标准”,RT-PCR基因分型总符合率为93.9%(77/82),其中1型符合率为96.6%(57/59),非1型符合率为87.0%(20/23)。结论:测序仪是HCV患者基因分型的金标准,具有准确度高的特点,但是该测序过程繁琐,耗时长,而荧光RT-PCR仪同样是检测HCV基因型的重要方法,其一定程度上可以缩短检测时间,并且也具备较高的准确度。Objective:To explore the application of fluorescent RT-PCR and sequencer in the detection of hepatitis C virus(HCV)genotypes,and to provide the basis for clinical diagnosis.Methods:From January 2018 to February 2020,2mL peripheral venous blood of 86 patients with hepatitis C were collected,and the HCV genotypes were determined by fluorescent RT-PCR and sequencer respectively.Results:Fluorescence RT-PCR was used to classify the serum of 84 patients with hepatitis C virus,including 56 cases of genotype 1(66.7%)and 28 cases of non genotype 1(33.3%),with 2 cases of uncertain typing.A total of 82 cases were classified by sequencing machine,including 59 cases of genotype 1(70.2%),all of which were genotype 1b,and 23 cases of non genotype 1(27.3%),including 9 cases of genotype 2a,1 case of genotype 3b,2 cases of genotype 4a,2 cases of genotype 4k,and 9 cases of genotype 6a.The typing of 4 cases is uncertain.Based on the sequencing results as the"gold standard",the overall coincidence rate of RT-PCR genotyping was 93.9%(77/82).The compliance rate for type 1 was 96.6%(57/59),while the compliance rate for non type 1 was 87.0%(20/23).Conclusion:Sequencer is the gold standard of HCV genotyping,with high accuracy,but the sequencing process is cumbersome and time-consuming,and fluorescent RT-PCR is also an important method to detect HCV genotypes,which can shorten the detection time to a certain extent,and also has high accuracy.
